The agency did not follow disciplinary process to ensure that staff sign in and out records were completed appropriately. — Visit: Complaint Investigation,Technical Assistance — Action taken: Agency administration will develop two new policies to address appropriate break time procedures as well as staff members' daily clock in and out procedrures. Agency administration will discuss the new policies with each staff member and will obtain signatures from each person signaling that the policies have been reviewed and understood as discussed. These signatures will be emailed to Licensing Consultant Donna Freeman upon completion.
