It was observed that a propane tank and lighter fluid bottle were on the ground and accessible to children in the outdoor play space. In the foyer by the front door, it was observed that multiple toxins including bottled cleaner & dish soap were on floor accessible to children. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Upon inspection of the children's files, it was found that a copy of the initial emergency contact information and agreement forms were not signed for Child #1, and not on file Child #2, CHILD #3, CHILD #4, AND CHILD #5. Upon inspection of the children's files, it was found that a copy of the initial emergency contact information for child #1 did not include information on a disability. Requirement: Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant of CRNP, which is needed in an emergency situation. Upon inspection of the facility fire drill log, it was found that drills were not conducted as of 12.1.2025. Requirement: Fire drills are conducted at least once every 60 days. It was observed that Evacuation maps were not posted in a conspicuous location on the 2nd and 3rd floor (living spaces) of the home. Upon inspection of the facility fire drill logs, it was found that smoke detectors tests were not conducted at least every 30 days. (CORRECTED ON SITE) Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Outdoor equip.- protective surfacing (3290.102(c))

During the time of inspection, it was observed that the Step 2 outdoor play equipment was not anchored into the ground. Documentation of the safety manual for the outdoor play equipment on file, states that the equipment should be anchored into the ground. The Outdoor Home Playground Safety Handbook by The Consumer Product Safety Commission states that there must be at least 9 inches of playground surfacing materials ; such as shredded bark mulch, wood chips, fine sand or fine gravel are considered to be acceptable shock absorbing surfaces while in use. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: I will use a gate to ensure that children will not play on the Step 2 playground. Inspection: Initial review.
