Upon inspection of the facility fire safety records, it was found that the following smoke alarm test dates exceeded the maximum of 30-days between required tests: 12.6.25/01.06.26 [32d]. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: The issue regarding the smoke alarm testing gates has been corrected and brought into compliance as required. Inspection: Renewal. Child #2 is missing the address for the release persons on their emergency contact form. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Child #1 is missing written consent signed by a parent for emergency medical care on their emergency contact form.
