On 4/29/26 at approximately 10:00am, Staff #1 arrived to take over the child care duties in the classroom. Shortly after arriving, Staff #1 began lining the children up to take them outside. At that time, it was discovered that child #1, who was now being supervised by staff #1, opened the gate of the second-floor classroom, went down the stairs, and exited the door to the driveway and attached parking lot of the business next door. The child ran onto Penn Ave before a customer at the store next door retrieved him and returned him to the facility. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times and staff shall be physically present with the children in their group on the facility premises and on facility excursions off the facility premises. This portion of the plan must have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a date for when this training will be completed. 3. The Certification Representative will conduct unannounced monitoring visits over the next three months. During this time period the facility shall have no occurrences of non-compliance relating to supervision. The correction date for this portion of the plan shall be no sooner than 8/7/26. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must develop a written supervision policy and procedure to be implemented in the facility. The written policy must address all subsections of 3270.113. The legal entity will submit policy/procedure to the Northeast Regional Office for approval. All staff must be trained on the approved policy and procedure and it must be documented in their files. The operator shall provide a date for when this will be completed. This is in conflict with the stated policy for parents to park and bring the children into the facility. Correction: As part of our ongoing commitment to providing a safe and efficient environment for children, we have modified our current childcare policy to include a new designated drop off and pick up location.
