Observed multiple torn areas, which create potential tripping hazards, in the blue matting of the outdoor preschool play area. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Both classrooms 124I and 124H have new plastic covers that eliminated the bolts around the bottom of the toilet areas. Photos of the correction have been submitted to rep for review.
