The child files for Child #1 and Child #2 were observed to contain immunization exemptions there were deemed unacceptable as they were on a form the provider gave to the parent to fill in which has not been acceptable since December 20th, 2022. Requirement: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record. Correction: parents have provided in writing on their on accord that their children are exempt from immunizations Inspection: Renewal. Refrigerator (3270.107)
The digital thermometer inside of the refrigerator located in the Kitchen area was observed to not be operable at time of inspection. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: batteries were replaced during inspection and thermometer was immediately operable Inspection: Renewal. The following ceiling tiles were observed to not be kept clean as evidence by having water spots from a previous leak that has since been repaired: two ceiling tiles in the kitchen area above the window at the top of the steps, one ceiling tile in the School Age room, and two ceiling tiles inside the left bathroom in the Older Toddler Room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
