Staff evaluations (3270.34(a)(6))

At the time of the Renewal Inspection conducted on 03/19/2026, the files for Staff Person #1, Staff Person #2 and Staff Person #3 did not contain a written staff evaluation completed within 12 months. Requirement: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months. Correction: Provider will complete staff evaulations. Initial health report for older toddler/preschool no more than 1 year/Initial and subsequent health reports (3270.131(a)(3)/3270.182(1))

At the time of the Renewal Inspection conducted on 03/19/2026, the file for Child #1, enrolled on 09/07/2025 did not contain an Initial Health Assessment. Requirement: The initial health report for an older toddler or preschool child shall be dated no more than 1 year prior to the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports. Correction: Provider will obtain copy of medical report from parents. Inspection: Renewal. Emergency plan (3270.27(a)(5))

At the time of the Renewal Inspection conducted on 03/19/2026, the Facility's Emergency Plan did not contain accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions. Requirement: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. Correction: Provider added addendum, stating that all children with special needs, chronic medical conditions and or disabilities will have extra accommodations to ensure their safety during an emergency. Age and Training (3270.31(e))

At the time of the Renewal Inspection conducted on 03/19/2026, the file for Staff Person #2 did not contain verification of the completion of 12 clock hours of childcare training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Correction: Staff Person #2 will complete the required 12 clock hours of childcare training. No business simultaneous use (3270.61(f))

At the time of the Renewal Inspection conducted on 03/24/2025, the previously measured child care space in rear classrooms of the facility was being utilized as storage space. Requirement: Indoor space in which children are receiving care may not be used simultaneously for other business, commercial, social or another purpose unrelated to the child care being offered. Correction: Items that are being stored in the classrooms will be removed. Since the removal will not take place before 05/09/2025 when current license expires, a temporary reduction in license capacity will need to occur. Inspection: Renewal. Toddler/preschool: updated health report every 12 months (3270.131(b)(2))

At the time of the Renewal Inspection on 03/24/2025, the file for Child #3 did not contain an Updated Health Assessment. The most recent Health Assessment on file for Child #3 was dated 08/27/2023. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Correction: Parent confirmed that visit has been scheduled for 04/23/2025 at 3pm. Form will be received from parent after the visit. Inspection: Renewal. Consent for emergency medical care required prior to admission/Consent for administration of minor first-aid required prior to admission (3270.182(3)/3270.182(5))

At the time of the Renewal Inspection on 03/24/2025, the file for Child #1 and Child #2 did not contain signed parental consent for emergency medical care and consent for minor first aid. Requirement: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Correction: The parents reviewed the statements and gave consent with their initials on forms. A full signature was written next to the initials. Inspection: Renewal. At the time of the Renewal Inspection on 03/24/2025, the facility could not provide written verification that the Annual Emergency Drill has been conducted. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: Emergency drill was conducted on 06/17/2024 and was documented on the Fire Drill form. The drill was rewritten on the Emergency Drill log. Inspection: Renewal.
