A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Age and Training/Health and Safety Training - Pediatric First Aid and CPR (3270.31(e)(4)(i)/3270.31(f)(10))

At the time of the Unannounced Inspection on 09/30/2024, the individual record for Staff Person #2 did not contain verification of the completion of the required Pediatric First Aid and Cardiopulmonary Resuscitation training. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Correction: Staff #2 had first aid training complete. At the time of the Unannounced Inspection on 09/30/2024, the individual record for Staff #2and Staff #3 did not contain verification of Emergency Plan Training. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: Staff Person #2 and Staff Person #3 had emergency preparedness training on their first day. Certificates were not placed in files at the time. Restraints prohibited (3270.113(e))

At the time of the complaint investigation on 09/30/2024, Staff Person #1 prevented Child #1 from leaving the book nook area by wrapping his arms around her torso, and using arm to block Child #1 from leaving book nook area Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: On 9/30/2024 upon the notice of concern staff member #1 was immediately placed on administrative leave. Following an internal investigation and the findings summarized in this inspection summary, staff member #1 was terminated on 10/28/2024. Staff member #1 remained on administrative leave for the duration of the investigative process through the termination date noted.
