At the time of the Renewal Inspection conducted on 08/14/2025, the file for Staff Person #1 did not contain two written references attesting to the individual's suitability to serve as a facility person. Requirement: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person. Physical examination/Information re: medical problems (3270.151(c)(1)/3270.151(c)(4))

At the time of the Renewal Inspection conducted on 08/14/2025, the file for Staff Person #2 did not contain a completed Health Assessment. The answers to questions 1,2 and 3 on the health assessment were left blank. Requirement: An adult health assessment must include a physical examination. An adult health assessment must include information on medical problems that might threaten the health of the children or prohibit a staff person from providing adequate care to children. Correction: Staff Person #2 scheduled a new appointment to get a new physical completed on 10/02/2025. Staff Person #2 will try to get an earlier appointment. Inspection: Renewal. No peeling paint or plaster (3270.77(a))

At the time of the Renewal Inspection conducted on 08/14/2025, there was peeling paint on the wall in the back of the Two's Classroom near the window. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Pertinent Laws & Regulations/General Health and Safety (3270.14/3270.21)

At the time of the Renewal Inspection conducted on 08/14/2025, Staff Person #2, hired 06/23/2025, was observed working alone. Staff Person #1's file did not contain verification of the completion of Pediatric First Aid and Cardiopulmonary Resuscitation Training or verification of the completion of the required Health and Safety Trainings. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: Staff Person #1 obtained the required Health and Safety trainings. Small Toys and Objects (3270.103)

At the time of the unannounced monitoring inspection on 09/16/2024, the outdoor play space designated for Toddlers was observed to have tiny rocks with a diameter measuring less than 1 inch throughout the play space. Child #1 was observed with a rock in their mouth, and Child #2 was walking around with a rock in their hand. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The provider will remove tiny rocks and any other objects that have a diameter less than 1 inch from the play space, ensuring that the play space is safe for use.
