The file for Staff person #2 does not include verification of emergency plan training completed within 90 days of hire (See LIS Code Sheet); the training was completed on 1.9.26. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. There was no lidded trashcan in the school age room which contained plastic forks, cups, etc., that the operator reported were trash products from breakfast. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. The hot water at the sinks in the boys' and girls' bathrooms in the preschool classroom exceeded 110° F. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Training chair - sanitized (3270.82(d))

In the boy's bathroom in the preschool room there was a red training chair that had a dried substance in the bowl and did not appear to be clean and sanitized. Requirement: A training chair is not a flushing toilet. A training chair shall be emptied and sanitized after each use. An acceptable sanitizing solution is 1/4 cup of bleach combined with 1 gallon of water. A sanitizing solution shall be treated as a toxic. Toilet and sinks proper height (3270.82(e))

There were no steps or stool at the sink in the Young Toddler area for able-bodied children to reach the sink for handwashing. Requirement: Toilets and sinks shall be at proper heights for children using them or shall be easily approached by means of platforms or steps. Clean and good repair (3270.102(a))

There were cracked Magna Tiles in the Main classroom that had cracked and broken edges with missing pieces of plastic on the edges. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Services proceeded (3270.123(a)(3))

The most recent Child Service Report on file for Child #4 is dated 1.8.26, more than 6 months after the previous Child Service Report, dated 5.21.25. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). The emergency contact form on file for Child #2 does not include the work address and/or telephone number for the enrolling parent. The classrooms and gym did not contain the emergency contact forms for the children receiving care in the spaces but only contained a list of telephone numbers. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: The provider replaced the master list of enrollment information and put the emergency contact forms in the classrooms as requested. Inspection: Renewal. Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports (3270.131(b)(2)/3270.182(1))

The two most recent health assessments on file for Child #1, who is a preschool age child, are dated 12.18.24 and 1.11.26, more than 12 months apart. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: Child #1 has a current health assessment on file. The provider will remind parents that the assessments need to be no more than 12 months apart. Parent replied that this was the first available date due to insurance reasons and being out of town for the holidays. Inspection: Renewal. Free from contagious/communicable disease (3270.131(d)(7))

The current health assessment on file for Child #1 does not indicate if the child is free from contagious disease and is able to participate in childcare. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Correction: The parent was notified that the information was not provided and that a new assessment needed to be provided. This child is leaving the program, so a new assessment will not be provided. The facility had an Albuterol inhaler labeled for use by a specific child that did not have written parental consent to issue. Requirement: A parent shall provide written consent for administration of medication or a special diet. Correction: The provider returned the Albuterol inhaler to the parent because there was not written consent in the childs chart. Inspection: Renewal. Disposable diapers (3270.135(a)(3))

A diaper change was observed in the Main classroom and the staff person was observed placing the soiled diaper on the changing table after removing it from the child, rather than immediately disposing of the diaper into a plastic-lined hands-free can. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: The staff person was informed of the violation and will throw the diaper away immediately after changing. The child was grabbing at the diaper so it was moved to the side to complete cleaning the child before it was thrown away. Inspection: Renewal. Written statement (3270.166(1))

The facility did not have feeding plans submitted in writing by the parent for the infants receiving care in the infant room. Requirement: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent. Correction: The feeding schedule was removed to be updated and was not placed back in the infant room. The staff had written instructions on a whiteboard, but a new schedule has been completed and placed back in the room. Inspection: Renewal. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. There was an unlocked door in the school-age room that opened to a room that contained an electrical box, which is accessible to children. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. The temperature in the school age classroom measured 60° Fahrenheit and the temperature in the gym measured 61° Fahrenheit, where children were observed receiving care.
