The facility has a valid reason the operator is unable to test the smoke detectors. The documentation of the most recent completed professional fire alarm inspection, required annually, was dated 2/21/25. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Upon inspection, the facility ensured that a working smoke detector was installed and in place to maintain fire safety coverage until the professional fire alarm inspection could be completed. Bottles labeled (3270.166(4))

There was a bottle in the infant room not labeled with the child's name. Requirement: Disposable nursers and bottles shall be labeled with the child's name. Correction: Upon identification of the unlabeled bottle in the infant room the bottle was immediately labeled with the child's name to ensure proper identification. Building Surfaces (3270.76)

There were two exposed bolts on the door to the restroom in the Pre-K room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The area was secured to prevent child access and maintenance was notified right away. The exposed bolts were removed and properly covered. To ensure no sharp or hazardous surfaces remain. Comply with CPSL/CPSL information (3270.32(a)/3270.192(4))

The signed disclosure statements on file for Facility Persons #1, #2, #3 and #4, all hired after 2/1/25, were not the most current version of the disclosure statement, required to be signed by facility persons hired beginning 2/1/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Persons #1-#4 may not work in a child care position at the facility without the correct signed disclosure statement on file. Correction: Provider reviewed all staff files and identified that staff #1,#2,#3,#4 had outdated disclosure statements on file. The most current version of the disclosure statement was immediately obtained and provided to each staff member. All identified staff completed and signed the updated disclosure statements, and the documents were placed in their file. Inspection: Renewal. Infant sleep position (3270.119)

There was an infant asleep in a bouncer seat in the infant room. The infant was not placed in the sleeping position recommended by the AAP. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. The current criminal history and child abuse clearances on file for Facility Person #1 were completed more than 60 months after the previous respective clearances on file. The current criminal history clearance on file for Facility Person #2 was completed more than 60 months after the previous criminal history clearance on file. The durrent mandated reporter training completed and documented on file for Facility Person #2 was completed more than 60 months after the previous training documented and on file. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: The criminal history and child abuse for staff member #1 12/10/24 and 12/17/24, respectively. The criminal history for staff member #2 was completed 1/27/25. Inspection: Renewal. Infant: updated health report every 6 months (3270.131(b)(1))

The current health assessment on file for child #1, a young toddler, was completed more than 6 months prior. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Correction: The health assessment was completed on March 3rd, 2025. The document was submitted to the inspector on March 10, 2025. Inspection: Renewal. Immunization record/Dismissal policy (3270.131(d)(5)/3270.131(e)(3))

The immunization record on file for child #1 does not indicate they have sufficient doses of the pneumococcal vaccine based on their age, as recommended by the ACIP. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Correction: The immunization was completed on 3/3/25. The record was shone to the inspector on 3/6/25. Toothbrush bristles up (3270.134(f))

The toothbrushes used by the children in the young toddler room were not stored exposed to circulating air. Requirement: Toothbrushes shall be stored with the bristles up and exposed to circulating air. Correction: A hygienic toothbrush holder with adequate air circulation was installed in the classroom for children's toothbrushes to be stored after each use. Inspection: Renewal. Diapering area not used for food prep (3270.135(c))

There were drinking cups used by the children next to the sink used for handwashing after diapering in the young toddler room. Requirement: The diapering area may not be used for food preparation or food service. Correction: Drinking cups were immediately removed from the sink and placed in a bin (labeled cups) on a shelf away from the sink. Inspection: Renewal. The operator indicates they had another inspection on 3/5/25, but they have been unable to obtain documentation of the inspection to this point. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: The fire alarm report has been submitted to the inspector 3/12/2025. On the report that was submitted, there was one deficiency that was cited. This deficiency was addressed by the building management team. The documentation of the deficiency was submitted to the inspector on 3/31/25. Inspection: Renewal.
