At the time of the Renewal Inspection, conducted on 01/23/2026, the trashcans located in the bathrooms of the facility did not have lids. At the time of the Renewal Inspection, conducted on 01/23/2026, evacuation routes were not posted in a conspicuous location on each floor of the facility. At the time of the Renewal Inspection 01/09/2025, the file for Staff Person #1 did not contain verification of the completion of annual Fire Safety Training. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.
