    Supervised at all times (3270.113(a))

When the CR entered the facility at 8:45 AM, supervision and ratio were assessed. The CR counted 10 children in the main classroom despite observing 11 children enter the building. The 11th child, a preschool child, was then observed sleeping alone on the other side of an L-shaped, solid, vinyl fence. The L-shaped, solid, vinyl fence was approximately 4 feet tall. The preschool child was sleeping between the two sides of the L-shaped, solid, vinyl fence. All three staff in their current locations could not see through the L-shaped, solid, vinyl fence in order to supervise the sleeping preschool child. No staff member was able to see, direct, or assess the sleeping preschool aged child. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Effective immediately, classroom layout and supervision practices have been adjusted to ensure all children remain within direct line of sight of staff at all times. The area that obstructed visibility has been rearranged/removed to eliminate blind spots. Staff have been instructed that children may not be placed in any area where they are not visible and directly supervised. Inspection: Unannounced Monitoring.
