A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency is not displayed in every child care space. Requirement: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children. Correction: I have come up with a written plan in the event of an emergency involving one of my children. It simply states that: In the event of a medical emergency involving your child, I will accompany the child to the emergency room, whether it be in an ambulance or my car. M husband, will contacted to return home to care for the other children. Either my husband or myself will reach out to the emergency contact person listed to let them know of the situation. Review policies and procedures (3290.121(a))

The facility does not provide a supervision policy and a dismissal policy in writing to the parents at the time of enrollment. Requirement: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. Updated infant health report: every 6 months/Initial and subsequent health assessments (3290.131(b)(1)/3290.182(1))

Documentation of an updated health assessment was not on file for child # 1. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record must contain initial and subsequent health reports. Correction: Have the parents get an updated health assessment form for me to keep on file. Inspection: Renewal. Small Toys and Objects (3290.103)

In the outside area, mulch and landscaping rocks were easily accessible to children. Pieces were less than 1 inch in diameter and presented a choking hazard to toddlers. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Unsafe Areas in Outdoor Space (3290.61)

In the outside area, the stair well leading downstairs to a basement entrance was not adequately restricted. The barrier in place was not secure and was easily moved. Requirement: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Comply with CPSL (3290.32(a))

Staff Person # 1 had mandated reporter training on file, however, it was not an approved training curriculum. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Retake the training and provide documentation to DHS.
