Operator or designated staff responsibility (3290.95(b))

Cert Rep noted that the fire detection log indicated there were fire detection device tests completed on 12/8/25 and then on 1/8/26. This is a span of 31 days, fire detection device testing was not completed every 30 days, or less, as required. Requirement: The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: I (Operator) made a fire detection device testing date schedule for the 2026 year and stayed within the 30-day timeframe. Inspection: Allocated Unannounced Monitoring. Clean, age appropriate (3290.105(a))

Cert Rep observed that the pack-n-play and cots were not labeled with the names of the children who used them. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Correction: put name on masking tape & attached to cot and/or pack n play. Cert Rep reviewed the Fire Detection Device Log and noted between the test dates of 5/9/25 and 6/9/25, 31 days elapsed. Fire Detection test was not completed timely. Requirement: The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met. Services provided (3290.123(a)(3))

Child #1, #2, and #3 did not have documentation of a child service report that was updated every 6 months. Documentation of the dates observed were 8/13/2023 followed by 7/01/2024. Requirement: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e). Child's hands washed (3290.134(a))

A child's hands were observed to be wiped not washed after being diapered. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: I will wash after diaper change.
