Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). Observed mouse excrement in the lower kitchen cabinet of the infant room. Requirement: Evidence of infestation of insects or rodents in the facility is not permitted. Correction: Staff member cleaned out and disinfected the dropping from the 1 cabinet the dropping were found in. Mouse droppings were found in one cabinet in the baby room on 1/9/26. The teacher immediately cleaned the whole cabinet with Clorox. I will call Orkin to implement a plan to visit and check all areas every 2 weeks. The baby room teachers will also check cabinets daily. Observed a broken table, which creates a potential toppling hazard, propped against the wall across from the PS bathroom. Observed a broken ceiling tile above one toilet in the PreK bathroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The lower cabinet was cleaned and disinfected. The area between the partition and window was cleaned. The table that was missing a leg was removed out of the hallway. Observed a toy shelf blocking the doorway between the infant and PS rooms. The facility does not have documentation of a passing fire system inspection conducted within the past 12 months. The most recent fire system inspection, conducted on 6/7/24 notes failures in the following areas: Alarm panel power, booster power supplies, initiating devices, indicating devices, and elevator recall. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Director contacted the building concierge (Paul) to see if the items that failed on the last inspection were corrected. Paul called Silco, who inspected the system, and had to leave a message and has not had a return call back yet. The building was sold to new owners in January 2025. The director reached out to the new owners for answers and hasn't heard anything yet, Paul said the new owners will be in on Monday and Tuesday (4/28 and 4/29) of next week so the director will speak to them directly so see that these items are fixed within 30 days.
