At the time of inspection, the medication observed for child #5 did not have the original container with the prescription label identifying whom the medication was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: The director will contact the parent of child #5 to obtain the label on the original container identifying the name of the medication and the name of the child for whom the medication is intended. At the time of inspection, the facility did not have a completed medication log for child #5's prescription medication. Requirement: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered. Correction: The director will establish a medication log for child #5's medication that includes the parent's signature authorizing the use of the medication. The medication log must identify the name of the medication, the name of whom the medication is intended and admission dates (to and from dates). Inspection: Renewal. At the time of inspection, the file for staff member #1 employed over 45 days contained a volunteer PA Child Abuse History clearance. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. Correction: Staff member #1 will not work in a child care position at the facility until the completed PA Child Abuse History clearance requested for the purpose of employment has been submitted to the facility's files. The director will ensure that staff member #1 completes a request for a PA Child Abuse History clearance requested for the purpose of employment.
