Correction: On 3-18-26 the staff person's transcript was obtained and placed in her file as well as sent to the licensing rep for verification of credit hours in order to correct this violation. The financial agreement form in the files for children #2 and #3 were not signed by the parent. Requirement: An agreement shall be signed by the operator and the parent. Correction: The financial agreement documents were handed to the parents on 3-25-26 to ensure all documents contained the signatures required for each document to remain in compliance with this regulation. The emergency contact form in the files for children #3 and #5 did not contain the enrolling parent's work address and telephone number. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: The emergency contact documents were handed to the parents/guardian on 3-25-26 to ensure all documents contained the addresses and phone numbers of the enrolling adult required for each document to remain in compliance with this regulation. Emergency contact information was not present for children observed to be receiving care in the Infant room. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: The director collected the emergency sheets from the office and the toddler room to replace them with the updated documents needed and copied the documents to have enough to store in each room. Each emergency contact document was updated (signatures and addresses) then copied and placed in each room that serves children on 3-27-26 to be in compliance with this regulation. A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not posted in the Infant room, Toddler room, Preschool room, School-Age room, Back Yard, or Back Porch. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. Correction: On 3-26-26, the emergency transportation plan containing protocols for transporting children to emergency care along with staffing provisions was created and posted in each room of the facility as well as shared with the parents as a part of the emergency plans and procedures for the site. Facility files contained documentation of testing the fire detection system on 12/19/25 and 1/19/26, which exceeded 30 days between testings. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: The fire detection system document was reviewed and with the clear explanation of the regulation, the director was able to plan for drills within the required time frame. The dates beyond February were checked for 30 day compliance and we put a detection system alert in our calendar system to be reminded of the 30 days rather than rely on the dates as was done in the past. The first aid kit in the Infant, Toddler, Preschool, and School-Age rooms was observed to lack scissors, tweezers, soap, and tape. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: We purchased the first aid kits needed for each kit on 12-19-2025 to correct the violation of specific items being absent from the kits currently placed in each room and will replace the kits as the items arrive in the mail. Handwashing signs (3270.82(h))

A handwashing sign was not posted near the diapering area in the Infant room. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: The hand washing sign was printed, laminated and posted on the wall behind the changing table in the infant room to correct the violation of separate postings being posted for each sink and each diapering area. Refrigerator (3270.107)

An operating thermometer was not placed in the refrigerator in the School-Age room. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: We purchased a pack of thermometers to replace both the missing thermometer in the downstairs refrigerator as well as the current thermometer located in the kitchen refrigerator to have two newly updated thermometers in both refrigerators. The facility files did not include documentation of the proof of purchase of the facility's fire detection system. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: I contacted both Guardian Protection Services and ABC Fire Company to get information on the installation dates for the fire detection equipment/services at the property of 2404 Perrysville Avenue. I was given an approximate date/timeframe of March 2018 for the beginning of the interactions of the companies and the property. I completed and signed the necessary form for the attestation of fulfilling this mandate to the best of my knowledge. Inspection: Initial review.
