On 1/22/26 at approximately 10:30 am, observed staff #3 in the toddler room bathroom with 6 older toddlers. The open bathroom has one toilet, one sink and a changing table. The changing table is located along the wall by the door. Staff #3 was changing a toddler's diaper at the changing table. The staff was facing the wall with their back to the rest of the room. The staff was unable to supervise the other 5 toddlers crawling around on the bathroom floor. One toddler was playing under the sink pulling on the pipes and hoses. Staff # 3 was unable to see, hear, direct and assess the activities of the 5 toddlers on the bathroom floor. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Staff #3 was immediately counseled regarding active supervision requirements during bathroom and diapering routines. Effective immediately, staff will not take groups of children into the bathroom if adequate supervision cannot be maintained. Staffing arrangements will be adjusted so that one staff member supervises the group while another staff member completes diapering, or children will remain in the classroom under supervision until they can be safely rotated.
