In the Art Room of Preschool 1, a gray cracked bin was observed to be accessible to children and could cause injury. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. The file for Staff #3 lacked two written, nonfamily reference letters attesting to the person's suitability to serve as a facility person. Requirement: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person. 65º F minimum (3270.70(a))

On 10/23/25 at 12:09pm, the indoor thermostat at the facility was reading at 62 degrees. Requirement: The indoor temperature shall be at least 65° F. Correction: This was corrected the same day it occured. As soon as the Executive Director was notified of the problem she called several HVAC service companies until she found one that could come that same day. The HVAC company came out after hours around 7pm and fixed the faulty thermostat. The Executive Director was present for this. The heat immediately began running and the center was at 74 degrees at 6:30AM October 25, 2025, when the center opened. Inspection: Complaints- Legal Location. The facility's fire detection log contained documented dates 8/13/25-9/30/25, more than 30 days apart. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Health and Safety Training (3270.31(f))

Staff #1, hired for more than 90 days, did not obtain training in the 10 required health and safety training topics. Requirement: Staff #1 must complete PA Required Health and Safety for Centers-Group Child Care Part 1 and Part 2. Correction: The staff did not work with children until Part 1 was completed on Sept. 19, 2025. The staff will completed Part 2 by the end of September 2025. The file for Staff #2 contained a volunteer child abuse clearance, making it invalid. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Staff #2 -- may not work in a childcare position at the facility. Correction: Staff #2 applied for the correct CPSL clearance on the day of the visit. 9/10/25 She did not work with children until the certificate was received on 9/17/25. The file for Staff #1, an assistant group supervisor, included documentation of 2 years' experience with children, but lacked documentation of education. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. Correction: The staff member requested their High School transcripts on the day of the visit 9/10/25. They were received on 9/12/25. Inspection: Allocated Unannounced Monitoring. Locked or inaccessible (3270.66(a))

In the upstairs bathroom near the office, Lysol wipes, labeled "keep out of the reach of children," were observed in an unlocked cabinet under the sink and accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The wipes were immediately thrown away by an employee as the package was almost empty. It is our policy in accordance with 55 PA Code Chapter 3270.66(a) that no cleaning materials or toxic materials can be accessible to children. The cleaning staff were notified of this infraction and warned of the consequence of a second infraction. Inspection: Allocated Unannounced Monitoring. The file for Staff #2 includes a First Aid/CPR certificate dated 1/26/25, after the expiration date of 9/2024. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: When Admin noticed that Staff #2 had an out of date First Aid/CPR certificate, staff was immediately sent home. Admin then enrolled Staff #2 in the First Aid/CPR class that was available that counted toward the PQAS requirement. Correction: We did renew the clearances 1 month ahead of time, however they took a long time to return, causing them to be late. We have current clearances now. Inspection: Renewal. Above ground window opening restricted to 6" (3270.72(d))

A window in the first-floor older toddler room was observed to be higher than the ground level, unlocked, and not modified to limit the opening to 6 inches or fewer. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: We have purchased window wedge locks which prevent windows from opening too far. These will arrive this week and will be installed immediately. Inspection: Renewal. Building Surfaces (3270.76)

The carpet in the back of the indoor playroom was observed to be ripped and not in good repair. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Handwashing signs (3270.82(h))

In the second-floor older toddler room, the diapering area lacked a handwashing sign. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: Admin put a handwashing sign on the sink in the diapering area as soon as we were alerted to the need for it. Inspection: Renewal. The emergency contact form for Child #2 lacked signed parental consent for emergency medical care. Requirement: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Correction: The Admin Coordinator contacted the parents of Child #2 and asked them to come in the next day to sign the paperwork. They did and then it was complete. Inspection: Renewal. The emergency contact form for Child #2 lacked a complete address for the release persons. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: The Admin Coordinator contacted the parents immediately upon being told and asked them to come in to complete the form. They did and then it was corrected. Inspection: Renewal. The two most recent health reports on file for Child #3 (a young toddler) are dated 7/9/24 and 2/5/25, more than 6 months apart. (SEE LIS CODE SHEET) Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record shall contain initial and subsequent health reports. Correction: This family could not be seen by the doctor on time because the child was sick.
