We conducted the fire drill the next day children were present (Mon. 10/27). We understand the regulation that fire drills must be completed at least every 60 days and need to include children in attendance. Conditions at the facility may not pose a threat to the health or safety of the children. Cleaning and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The inspector determined the hand sanitizer in a child off-limits area between the two rooms, used by staff only, was non-compliant. The doors to this area were immediately locked. Also, hand sanitizer was not visible but discovered when a white board was removed by the inspector. Because all hand sanitizers were previously removed from rooms 1 and 2, this was not noticed during the daily facility check. They were immediately removed when discovered. The hand sanitizers from Room 2 were removed. Furthermore, the doors to the upstairs classroom and second floor of the facility are closed due to lack of use during the fall session. In the large upstairs meeting room, a bottle of hand sanitizer was observed on an enclosed area desk, and a bottle of disinfectant spray was discovered on a desk shelf area. Conditions at the facility may not pose a threat to the health or safety of the children.. Until such time as the required pre- service trainings are completed, Facility Persons #2 and #5 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff persons #2 and #5-, staff persons #2 and #5 may not work in a child-care position at the facility. Correction: Staff members 2 and #5 were always supervised immediately following the inspection as they awaited the upcoming CPR/First Aid training. Due to rising costs and staff accessibility issues associated with free training along with the impact on the SHCDP 2025 budget, paid CPR training sessions will be hosted by SHCDP twice a year. SHCDP Facility person #2 is a new hire and the AGS(Executive Director) was in the room until the inspector came. The AGS asked another person to fill in but that person ended up leaving the position during the inspection. Facility person #5 just returned to the organization for the fall programming. Both people are scheduled for training during our next session on January 14, 2026. We understand that Facility persons #2 and #5 must be supervised if they interact with children; and if no qualified staff person is available to supervise, Persons #2 and #5 may not work inside the facility nor transport them. Due to weather, this area is not used as much during the months of November-March. All the bolt coverings were recently replaced, and intact as observed during the inspection. However, the very bottom one was probably the result of deer and rabbits which often try to gain access through this entry gate of the fence. The AGS(Executive Director) was serving in the room until the inspector came. The AGS presented the ECF to the inspector but was not working in the room at the time due to the inspection. Correction: Facility person #5 went to [their] regularly scheduled doctor's appointment in December. [They] forgot the form to have the physician fill out. He called the office and they promised to send it to him after the holidays. To ensure his biennial assessment is completed in a timely manner, we again requested a completed form from the physician. We also spoke with said Facility person and staff during the annual January staff meeting and reminded them that they cannot wait until the last minute to renew certifications. They need to book appointments one month in advance to in this case, coincide with health insurance plans and the number of times they can have check-ups during the coverage year. The staff member with all certifications and designated to supervise #4 was nearby moving lunches and camp bags for transport. We learned that the disruption of the inspection was intimidating to the supervising staff member who opted to move out of the way of the two inspectors. Conditions at the facility may not pose a threat to the health or safety of the children.Cleaning and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The inspector determined toxins were accessible to children on the next to last day of summer camp. Prior to the inspector's arrival, children were preparing for an outing, and the timing of their departure was supposed to coincide with other staff members bringing water hoses and wood display boards, etc. from the always locked storage room. The storage closet was unlocked in anticipation of the entire group from rooms one and two being gone from the facility. Ice melt was moved to the front to allow access to dolly's located in the back. The door was immediately locked when the children's outing had to be cancelled. Regarding the lotions, Staff was gathering their bags for departure and the person designated to lead the children onto the vans placed her bag on a shelf in preparation for departure. The inspector went inside her bag that had undergarments and a shirt on top and found lotion. The staffer's bag was immediately moved to a secure area where it would be out of the reach of children. In the same manner the inspector proceeded to go through bags in cubbies. Because this was a designated park (sprinkler) day, as part of skin care and hygiene needs parents provided lotions and soaps for their children in their camp bags which were stored in cubbies away from where children were gathered in the rooms. The inspector went through the bags and discovered the items among towels and other water activity gear. We removed all lotions, soaps, and sunscreens from the bags as advised. The kitchenette has had these designed childcare doors (half-doors) for many years, which secure the area. There is no handle or way for children to grab the door and open it. Children cannot gain access inside the kitchenette area from the outside. See picture. Entry doors to the kitchenette are always closed. Cleaners were placed in the cabinet as staff was cleaning equipment and preparing for the outdoor Celebration. Upon the advisement of the inspector, locks were placed on the doors and cleaners were moved to a secure cabinet. These actions will be reviewed during future staff training to make sure cleaning and other toxic hygiene items such as lotions are kept in an area inaccessible to children. However, the signature on 6/2 was omitted because another sheet was being prepared for the new year in preparation for our annual independent Fire system inspection conducted by PFE/Cintas and the required emergency letters to County EMA officials. The current completed sheet which contains documentation of the fire drills was in a file to present to the independent fire alarm and equipment inspectors, who were scheduled to conduct the inspection a couple of days later. Correction: A plastic pencil box was observed in Room 1 with a crack that determined to be a potential pinch point. This occurred due to routine use. Each child or small group has their own pencil case. SHCDP observes the condition of its classrooms, toys, supplies on a daily basis and while we observed nothing amiss with the other 13 boxes, we missed this particular pencil box in question. The box was removed and replaced. Staff were re-trained during the school year orientation on August 16th to monitor toys, desks, supplies, pencil boxes, etc. to ensure there are no hazards to children. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: Facility Persons #2 and #3 no longer work for SHCDP. They were applicants and delayed in getting their health assessments/tb reading due to transportation issues and appointment scheduling with their physicians. As explained to the inspector, Facility Person #1 was not working in the facility at the time of inspection. He had just come back from his professional football season, and he was offering to assist with putting up tents and building a stage and boards, etc. for the kids displays for the next day's celebration. As the inspector witnessed, he said goodbye to me and left the building during the inspection. Also, he mostly works during the school year until his football training season begins. His health assessment was valid before he left the country and we knew that if he were to come back for the school year, he would have to get a new health assessment performed, which he did on August 27th for a September start back in the facility.
