The file for child #1 contains a current health report dated 11/6/25 that was obtain after 60 days following the first day of attendance at the facility. The child start date was 8/25/25. Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Correction: The classroom teacher's spoke with the parent to inform them that the child physical needed to be updated. At the time of this conversation the parent informed staff that the doctor's office couldn't schedule them in until 11/6/25, and would bring in an updated copy of the health report. The director thought that this was acceptable since the parent provided documentation from the doctor's office stating that the office could not fit the family in for an on time appointment. The restroom closed to the classroom 1st stall lacked washing hands sign. The restroom near the gym 1st stall lacked washing hands sign. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: At the time of inspection the PreK staff retrieved copies of hand washing signs from the classroom and replaced the missing sign in the restroom closing to the classroom and in the restroom near the gym Inspection: Renewal. Building Surfaces (3270.76)

There was a hole, damaged plaster on the right-side wall in the cafeteria/Gym. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The Director will work with the school Principal and other at the school to make sure that the school fixes the damage to the wall in the gym/cafeteria. Inspection: Renewal. There was a big black unlidded garbage can that contains contaminated by human secretions cups, cereal bowls, spoons, and milk cartons in the room. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: The director had the classroom lead teacher contact the food services director asking for them to not provide the big black unlidded garbage can for the classroom. Inspection: Renewal. There was a screwdriver in the pencil holder on top of the desk accessible to children. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The staff person present during the inspection visit immediately removed the screwdriver and placed up out of the reach of children in order to not pose a threat to the health or safety of the children. There was a broken plastic bin in the room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. The Aquaphor for child # 3 was expired. The expiration date was 9/24. Requirement: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label. Correction: The staff that was present in the classroom was informed by the director that the Aquaphor for child # 3 was expired. The staff informed the parent that we could no longer use this since it was expired. It was removed from the emergency bag and given back to the parent at pick-up. Inspection: Renewal. The file for staff #4 lacked a Child Abuse clearance and current CPR training. The operator was able to obtain a copy from a different location and place it in the file during the renewal inspection.
