On 6/9/26 a group of OT were observed playing in the playground. The playground is missing part of the fencing creating an unsafe area providing access to a parking lot. Requirement: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Correction: We contracted with a fencing company to install a new privacy fence once the new retaining wall is installed at the end of the month. There was a hand sanitizer bottle on top of a shelf in the SA room label " keep out of reach of children" accessible to children. There was a Chelidonium Majus plant considered toxic in the toddler's playground area accessible to children. There were four damaged ceiling tyles in the indoor play space. There were two broken metal fence pipes creating sharp edges and a bended fence on back of the preschool/SA playground. There was garbage in the playground area while the toddlers were using the playinground. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The 4 damaged tiles were replaced in the hallway. A new fence will be installed in the playground area. The fence will be installed by 8/14/2026. There was a cracked blue basket in the preschool room. There was a ripped blue couch with exposed foam in the SA room library area. There were 2 ripped puppets with exposed foam and a cracked playhouse door in the OT room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The blue basket was taped and then later disposed of. The blue couch in the library area was taped at the rip. The 2 ripped puppets were disposed of. "Based on staff statements, the child care facility was flooded on 1/3/26. Due to the damage caused by the flood, all the drywall at the bottom of the walls will be cut out and replaced. All of the flooring has been removed and will be replaced. The facility is working with a professional restoration company to make the repairs." Correction: On 1/5/26 parents were called to pick up the children due to the water main break.
