In the PreK room, children were observed to use hand sanitizer prior to eating snack and did not wash their hands with soap and water. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: Childrens hands will be washed with soap and water before all meals and snacks and after toileting and diapering. The most recent health reports in the file for Child #1, a preschool child, exceeded 12 months between reports. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: The health report was completed and filed based on office availability and the insurance policies for coverage. In the Young Preschool room, a bin containing children's dolls was not in good repair as it was observed to be cracked on the side and had a broken handle. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Stairs, exits, etc. unobstructed (3270.91(a))

Inside Room 8, the stairwell emergency exit door was blocked by cabinets placed directly in front of it, preventing the door from being opened from within the classroom. On the hallway side: When viewed from the hallway, the same exit door was also visibly blocked by cabinets directly in front of the door, it could not be opened from the hallway side either. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: The fire marshal will review the building and determing if 2 means of egress are needed. The letter will be kept on file. If two means of egress are needed, all furniture, shelving, cabinetry, etc will be removed and the doors will be accessible and clear of any obstructions. Inspection: Renewal. Observed peeling, flaking paint and rust on the iron gate of the outdoor play space. In the Young Preschool room, a large tear was observed in a window screen of a window that staff identified is opened regularly for ventilation. Requirement: Screens shall be in good repair. Contaminated trash in closed plastic-lined receptacle/Lidded waste receptacles (3270.67(d)/3270.82(i))

In the children's restroom in the Gross Motor room, a trash receptacle contained used tissues and was not plastic-lined and did not have a closed lid. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle. Correction: Trash recepticle was removed. Inspection: Renewal. Health and Safety Training (3270.31(f))

The Health and Safety Training (Part Two) in the file for Staff Person #1 was conducted more than 90 days after the staff person's first day working in child care (See LIS Code Sheet). Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. In the Young Preschool Room, two blue children's cushions in the Calming Corner were observed to be ripping on the corners and were not in good repair. In the Toddler Room, a children's wooden play sink was observed to have a cracked handle with sharp edges, and another handle was observed to have broken off leaving a nail protruding from the play sink. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The ripped cushions were removed from the childcare space and disposed of. The cracked handle and exposed screw were covered with a protective padding to eliminate sharpness and a firm plastic barrier. Inspection: Allocated Unannounced Monitoring. An infant child was observed to be sleeping in a crib that lacked 2 feet of space on three sides in the Infant Room. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: The child was removed from the crib. The facility's documentation of testing the fire detection system exceeded 30 days between testing from 7/17/24 to 9/13/24, 9/13/24 to 11/6/24, and 11/6/24 to 12/10/24. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.
