The file for Staff #1 contained Mandated Reporter Training that had been updated in an interval greater than 60 months. This is evidenced by the previously documented mandated reporter training on file being dated 2/4/21 and the update being dated 2/19/26. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: I have submitted (Staff #1's) updated Mandated Reporter Training Certificate, but will make sure that in the future all updated Mandated Reporter trainings aree updated within the 60 months required time frame Inspection: Allocated Unannounced Monitoring. Exam communicable disease (3270.151(c)(3))

The file for Staff #1 contained a Health Assessment that stated they had a communicable disease. There was no accompanying documentation stating the condition and the risk it might pose to others exposed to this individual. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: Staff #1 took her health form to her doctor that evening (2/10/26). The doctor checked the communicable box as yes in error and immediately correted staff #1's health form by checking "no" to the communicable disease question. Child #3 had an expired Ibuprofen in the classroom. Requirement: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label. Immunization record/Exemption documentation from parent/guardian (3270.131(d)(5)/3270.131(e)(1))

The file for Child #2, enrolled for more that 60 days at the facility, did not contain documentation of a 2nd and 3rd dose of the DTAP vaccination, 2nd dose of the HIB vaccination, 2nd dose of the Polio vaccination, and a 2nd and 3rd dose of the Pneumococcal vaccination as required based on the child's age as per the ACIP recommended schedule. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in childcare group settings). Child #3 must be dismissed from care by close of business 2/10/26 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. Correction: The parent of child#2 was contacted immediately and she provided the center with the required documentation of the 2nd and 3rd doses of the DTAP vaccination, the second dose of the HIB vaccine, the second dose of the Polio vaccine and the 2nd and 3rd doses of the Pneumococcal vaccine. Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports (3270.131(b)(2)/3270.182(1))

The file for Child #2, a preschooler, contained health reports updated in an interval greater than 12 months. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: There was a lapse in child #2's health report. An updated health form is now in the child's file Inspection: Renewal. Handwashing signs (3270.82(h))

The sink and diapering area in the Toddler 2 Room lacked hand washing signs. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. The first aid kit located in the Toddler 1 Room lacked sterile gauze pads. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: Gauze pads were immediately placed in the first aid kit in the Toddler 1 room, along with a list of needed items. Inspection: Renewal. Above ground window opening restricted to 6" (3270.72(d))

Observed a window in the School Age 2 Room that was accessible to children and was not modified to limit the opening to 6 inches or fewer. Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: The window in the School Age 2 room is now limited to an opening of 6 inches. This was corrected immediately. Inspection: Renewal. General Health and Safety (3270.21)

The loose cord running from the electrical outlet to the air purifier in Toddler 2 created a tripping hazard to the children in care. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The loose cord running from the outlet to the air purifier in Toddler 2 was immediately secured to the wall to prevent this tripping hazard.
