    The file for Staff #1 contained a Health Assessment that stated they had a communicable disease. There was no accompanying documentation stating the condition and the risk it might pose to others exposed to this individual. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. Correction: Staff #1 took her health form to her doctor that evening (2/10/26). The doctor checked the communicable box as yes in error and immediately correted staff #1's health form by checking "no" to the communicable disease question.     The file for Child #2, enrolled for more that 60 days at the facility, did not contain documentation of a 2nd and 3rd dose of the DTAP vaccination, 2nd dose of the HIB vaccination, 2nd dose of the Polio vaccination, and a 2nd and 3rd dose of the Pneumococcal vaccination as required based on the child's age as per the ACIP recommended schedule. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in childcare group settings). Child #3 must be dismissed from care by close of business 2/10/26 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. Correction: The parent of child#2 was contacted immediately and she provided the center with the required documentation of the 2nd and 3rd doses of the DTAP vaccination, the second dose of the HIB vaccine, the second dose of the Polio vaccine and the 2nd and 3rd doses of the Pneumococcal vaccine.
