The fire detection system was not tested every 30 days as evidenced by the following test dates:12/3/24 -1/7/25, 3/31/25 - 5/6/25, 7/1/25 - 8/5/25,9/2/25 - 10/7/25. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Fire detection system will be tested every 30 days or sooner to ensure the system is working correctly. No peeling paint or plaster (3270.77(a))

Observed peeling paint on the church building wall in the playground (large muscle space). Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Wall surface under windows were scraped and repaired. Staff evaluations (3270.34(a)(6))

The evaluation for staff #3 was late. It was due 8/2025 and completed 9/2025. Requirement: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months. Correction: - Staff #3 was due for an annual evaluation in August 2025. Staff received the evaluation in September 2025. Inspection: Renewal. The following staff updated clearances late (beyond 60 months): #1 - child abuse due 5/2025 and obtained 9/2025; state police due 5/2025 and obtained 9/2025; FBI due 6/2025 and obtained 9/2025. #2 - child abuse due 8/2025 and obtained 10/2025; state police due 7/2025 and obtained 9/2025; FBI due 8/2025 and obtained 9/2025. #6 - state police due 9/13/2025 and obtained 9/17/2025; FBI due 9/10/2025 and obtained 9/18/2025. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: Staff #1 became up to date as of 9/2025, staff #2 became up to date as of 10/2025, and #6 became up to date as of 9/2025. Inspection: Renewal. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Persons # 4 and #5 may not work in a child care position at the facility until valid clearances are obtained. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: Staff #4, from California- California does not release clearances for employment purposes. Facility has placed a note in the employee's file stating that Calif. Does not provide clearances. This was completed, as directed by our Certification Rep. Staff #5, from Florida, on 11/20/2025, SCCC received a letter that no criminal record was found in Florida. Both staff signed the updated Disclosure Statement on 11/17/2025. All necessary out of state clearances were obtained. Staff did not work with children until valid clearances were on file. Health and Safety Training (3270.31(f))

Staff person #4 has not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): (PQAS pediatric first aid and cpr). Requirement: Staff person(s) shall complete professional development in the topics of 3270.31(f)(1 -- 10) within 90 days of hire. Staff person #4 will have until _12/27/25 to complete the required training. Until such time as the required training has been completed, staff person #4 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff available to supervise staff person 4-, staff person #4 may not work in a child-care position at the facility. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: Staff members had health assessment performed beyond the 24 month date required. Each were a month past due. Staff were up to date within 30 days after the 24 month period. The file for staff #1 contains pediatric first aid and cpr training that was renewed late. It was due 6/2023 and obtained 8/2023. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Locked or inaccessible (3270.66(a))

Observed one unlocked janitor's closet in the hallway that had cleaning products in it that were accessible to the children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The closet door was locked on 11/19/24 by custodial staff and the child care director, and custodial staff was reminded to keep the janitor's closet locked at all times while children are in the center. Every 60 days (3270.94(a)(1))

On 2 occasions since the previous annual inspection, fire drills exceeded 60 days. ( 11/8/23-1/12/24; 3/11/24-6/21/24). Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Correction: Fire drills were past the 60 day time span but were completed. Administration will do a fire drill at least every 60 days. Inspection: Renewal. Age and Training/Exp, educ., training at facility (3270.31(e)/3270.192(2)(iii))

The file for staff #1 contains annual training hours for the 7/2023 - 7/2024 period that were late. 5 hours from 11/2024 were used to satisfy the 7/2023 - 7/2024 window. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.
