A renewal inspection was conducted on 12/1/2025. At that time, a sample of children's files were reviewed. Immunization summary on file for child #1 indicates that child has received 2 of 3 doses of the DTaP vaccine. Child #1 is an infant (see LIS code sheet for date of birth). There is no exemption on file. Immunization summary on file for child #3 indicates that child has received 1 of 2 doses of the Hep A vaccine. The first dose was administered on 11/25/2024. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Child #1 and child #3 must be dismissed from care by close of business 12/1/2025 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. Complaint investigation was conducted on 12/1/2025. At that time, minimal interaction between staff members and children was observed during investigation. Staff were not observed to be talking or playing with children in the toddler, preschool, and young school-age areas. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Infants and toddlers shall be provided stimulation by being held, rocked, talked to, played with and carried. TIERED LIS: 1.
