Staff person #1 has completed mandated reporter training within 90 days of their date of hire (see LIS code sheet); however, the mandated reporter training does not meet the minimum of three hours. Staff person #2 and #3 has completed mandated reporter training within 90 days of their date of hire (see LIS code sheet); however, the mandated reporter training is not intended for an appropriate target audience. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to Child Protective Services). Until such time as the required training has been completed, staff person #1, #2, and #3 must be supervised, when interacting with children at a minimum by, an (AGS) who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, #2, and #3, staff persons #1, #2, and #3 may not work in a child-care position at the facility. Correction: Staff person #1 completed the training on 12/11/2025 which was before the actual physical site inspection. Staff person #2 will complete the training prior to 1/19/2026 - which is their next scheduled date to work, as they are a casual staff. Staff person #3 completed training on 12/26/25 after being notified of the violation following the inspection at Child's Way. 110º F or less (3270.69(b))

In the School Age Classroom, the water temperature at the sink in the classroom was measured at 111 degrees. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: On 1/6/26 the hot water for the sink in the school age classroom was turned down and adjusted to 96 degrees to maintain a temperature under 110 degrees. Clean and good repair (3270.102(a))

The yellow car in the Outdoor Playspace was missing a strap in order to buckle the child in the toy posing a falling hazard to the children. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: On January 8, 2026 a seatbelt was ordered from the manufacturer of the yellow car at Gametime. The Director of Child's Way placed tape over the entrance/side of the yellow car so that it will remain out of service on the playground until installation of the new seat belt takes place. Designated release persons (3270.123(a)(5))

The Agreements in the files for Child #1, #2, #3, #4, and #5 lacked the persons designated by a parent to whom the child may be released. Requirement: An agreement shall specify the persons designated by a parent to whom the child may be released. Correction: On December 29, 2026 the following statement was added to the financial agreement: "See Emergency Contact Form to verify person(s) designated by caregiver to whom child may be released." Age and Training (3270.31(e))

The file for Staff #2, lacked documentation of 4 hours of the required 12 annual clock hours of childcare training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Correction: Previously at Child's Way ( medically fragile child care), nurses did not have the expectation of completing 12 hours of annual training. On Staff person #2's first day back to work, on January 19, 2026, staff will complete 4 hours of professional development to fulfill the 12 hour training requirement. Disposable nursers or parents (3270.166(3))

According to staff statements, the commercial dishwasher at the facility is not consistently being used to clean infant bottles. Bottles are currently not sent home and kept at the facility. Requirement: Disposable nursers shall be used for infants unless bottles are provided by the parent or unless a commercial dishwasher is used by the facility. Correction: Parents will be made aware at the time of enrollment that ALL bottles will be washed in the dishwasher for proper cleaning and disinfecting. Facility will ensure all bottles will be washed in the dishwasher after each use. Bottles that are brought in by families that have a valve that may melt when subjected to higher temperatures during cleaning, and therefore not able to be placed in the dishwasher, will be rinsed out here after it is used and sent home to be properly cleaned and disinfected. Pertinent Laws & Regulations/General Health and Safety (3270.14/3270.21)

The facilities Emergency Plan did not address Continuity of Operations. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The Facility Emergency Plan was updated to address Continuity of Operations. The following statements were added to section 3 C on page 4: 3) Any documents not in the electronic medical record will be taken with the patients during the evacuation. 4) We will be able to maintain continuity of operations as follows: - All finance records and documents, including payroll, are backed up off site and in the cloud, allowing continuation for paying employees and billing statements. - Medical records are in an electronic medical record that is backed up off site and in the cloud. The medical record is accessible via a web program that is secure with authentication. - All supplies can be ordered online by our material management. Inspection: Allocated Unannounced Monitoring. Every 60 days (3270.94(a)(1))

Upon review of the facility's Fire Drill log, the following tests were observed to have been conducted more than 60 days apart: 3/28/24-5/30/24 (63 days) and 7/24/24-9/25/24 (63 days). Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Correction: The Facilities Manager created a separate preventative maintenance schedule specific for Child's Way to conduct fire drills between 53 and 58 days to cover each day of the work week. If the date of the drill gets moved, then the Facilities Manager will go into the Preventative Maintenance Program and reschedule the following drills. Inspection: Allocated Unannounced Monitoring. Availability of certificate of compliance and applicable regulations (3270.25(a))

The operator did not provide the parents of each child enrolled with information on how to electronically access the regulations. Requirement: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents. The operator shall provide the parent of each child enrolled with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office. Correction: The Education Program Supervisor posted a laminated flyer on 10/24/24 to the bulletin board inside the entrance of Child's Way, which is used to post information for parents. The flyer includes the phone number of the Western Region Office of Child Development and Early Learning Bureau of Certification, the QR code to access 55 Pa. Code Chapter 3270, Child Care Centers, and the link to the website. In addition, the flyer was uploaded to the parental permissions in the FAMLY communication app on 11/5/24, which parents will need to acknowledge that they received. The phone number and link to the website were also added to the Child's Way Parent Handbook with an updated handbook uploaded to the FAMLY communication app on 11/5/24, which parents will also need to acknowledge that they received and reviewed.
