During an unannounced monitoring inspection on 4/17/26 Facility Person #1 did not have a PA Child Abuse Clearance on file. Facility Person #1 was observed providing care on 4/17/26. See LIS code sheet for Facility Person #1's first day with children. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 must be removed from a child care position with direct contact and routine interaction with children until all missing clearances are completed and on file at the facility. Correction: Facility Person #1 was suspended effective 4/17/26 until the clearance was obtained. Facility Person #1 obtained the child abuse clearance on 4/27/26 and returned to the center after handing in the clearance. The operator will comply with the CPSL. All employees will be required to have completed and verified clearances prior to their first day working with children. Administration will implement a strict pre-employment verification process, including a clearance checklist before scheduling staff. A centralized tracking system will be maintained to monitor clearance status and expiration dates. Monthly staff file audits will be conducted to ensure ongoing compliance. Health and Safety Training (3270.31(f))

During an announced monitoring inspection on 4/17/26 Staff person #2 has acceptable pediatric first aid/CPR training in an approved curriculum; however, the trainer was not PQAS-certified in that training topic at the time of training. Staff person 2 was observed to be providing care to children while supervised on 4/17/26. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10 or (g)1-10. Until such time as the required training has been completed, Staff person #2 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise Staff person #2, Staff person #2 may not work in a child-care position at the facility. Correction: Administration will verify that all trainings are completed through PQAS-certified instructors prior to acceptance. Training certificates will be reviewed and approved by the director before staff are allowed to work independently. Quarterly staff file audits will be conducted to ensure all professional development meets DHS and PQAS requirements. Staff person # 2 was not un supervised until receiving the training. Small Toys and Objects (3270.103)

During a compliant investigation on 4/17/26 Cert rep observed small 0.5 in. diameter rubber bands, 0.5 in. diameter pompoms and plastic bags on an accessible shelf as well as plastic bags, and a 0.25 in. diameter screw in an unlocked accessible desk drawer in the Preschool classroom (Orange Room 2nd floor). Staff working in this room reported they are aware that preschool aged Children 1 and 2 who were present and receiving care in this room were known to still be placing objects in their mouth since prior to 4/10/26. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: All small objects (rubber bands, pompoms, screws) and plastic bags were immediately removed from accessible areas in the preschool classroom. Staff reorganized materials and secured all hazardous items in locked drawers and elevated storage out of children's reach. A full classroom safety sweep was conducted the same day to ensure no additional hazards were present. Inspection: Complaints- Legal Location. During a renewal inspection on 11/7/25 Cert rep observed Desitin and Butt Paste in an accessible cubie in the Green room and A&D ointment in the unlocked cabinet under the changing table in the 1st floor Orange Room. Cert rep observed hand sanitizer in accessible drawer of the teacher desk in the 1st floor Orange Room, and insect jelly in the unlocked cabinet under the lizard enclosure. These items were all labeled "Keep out of reach of children." Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children. Group sup qualifications (3270.35(b))

During a renewal inspection on 11/7/25 Staff 1 had a high school diploma, a CDA, and 2 years experience working with children however Staff 1 did not have an associates or bachelors degree. Staff 1 has been working as a group supervisor. Requirement: A group supervisor shall have attained one of the qualification levels specified at §3270.35(b)(1)-(4). During a renewal inspection on 11/7/25 the parent to the parents regarding the facility emergency procedures did not include information regarding the lockdown procedures or accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: Director will add lockdown procedures or accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions to the parent letter for the emergency plan. During a renewal inspection on 11/7/25 Cert rep observed milk a child brought from home was kept in the classroom, and not refrigerated. Requirement: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated. During a renewal inspection on 11/7/25 Cert rep observed there were not scissors in the first aid kit in the white van whose license plate starts with MDG. This van is used for transportation of children, and no other first aid kit is taken which the staff when transporting children. Requirement: A first-aid kit, including the contents as specified in §3270.75 (relating to first-aid kit) shall be in the vehicle when children are being transported. During a renewal inspection on 12/9/24 Cert rep observed Hand sanitizer which is labeled keep out of reach of children was found in an unlocked accessible drawer in the Preschool 1 classroom. During a renewal inspection on 12/9/24 Cert rep observed peeling paint in the following locations: Preschool room 1(wall by the toy kitchen area), Preschool room 2(wall to the left of the Christmas tree), currently unused green room(wall between the first two windows from the door), and in the outdoor play space (wall under the stars Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: On the day of inspection peeling paint in the following areas was covered with tape. Director will cover/remove the peeling paint in the outdoor play space when children are not in care. During a renewal inspection on 12/9/24 Cert rep observed that the written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not accompanying staff when transporting children to and from school. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. During a renewal inspection on 12/9/24 cert rep observed the manufacturer's instructions for child safety restraints (booster seats) was not being kept in the vehicle. During an unannounced inspection on 11/19/24 Staff 1 only had one reference documented in her staff file. Staff 2 had a disclosure statement on file that is not dated by the staff. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).
