Based on a self-reported noncompliance and staff statements, on 6/17/26 at approximately 11:45am Staff person #1 was in the gross motor space with a group of older toddlers, when Child #1 required a diaper change. Staff #1 directed and observed Child #1 to go down the hallway to the Older Toddler classroom where the staff thought another staff was waiting to complete the diaper change. However, there was a miscommunication between the staff and there was not a staff in the Older Toddler classroom. Child #1 was unsupervised in the classroom for approximately two minutes. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Child was immediately placed back into a supervised environment. Staff was removed from the room to discuss the incident. Staff involved were placed on a suspension. Staff are assigned 36 hours of continuing education hours around supervision, safety, and professionalism. Staff are completing hourly name to face checks. Staff are using child ratio tags to ensure compliance. Staff meeting was held to discuss transitions and face to face hand off of children. Inspection: Complaints- Legal Location.
