Evacuation routes were not posted on either of the levels at the facility. Requirement: Evacuation routes are posted in a conspicuous location on each floor of the facility. Within 60 days/Initial and subsequent health assessments (3290.131(a)/3290.182(1))

The initial, Health Assessments in the files for Child #1, Child #2, and Child #3, were dated and provided more than 60 days following the first day of attendance at the facility. Requirement: The operator shall require the parent of an enrolled child to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record must contain initial and subsequent health reports. Correction: The health assessment for Child 1, Child 2, and Child 3 were a little late, but they are in the file. Comply with CPSL (3290.32(a))

The files for Staff #1 and Staff #2 contained documentation of NSOR Certificates dated 11.19.20 and 12.4.25, which exceeded the 60-month requirement. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Both NSORs were a little late, but they were received. Inspection: Renewal. General Health and Safety (3290.18)

There was no documentation of a Shaken Baby Policy. The Emergency Plan did not contain a section for Continuity of Operations. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: A new Shaken Baby Policy will be made, and a section will be added to the emergency plan - Continuity of Operations. Inspection: Renewal. Age and training/Required training (3290.31(f)/3290.192(4))

The file for Staff #1 did not contain documentation of 12 hours of child care training, as only three (3) hours were documented in the file from 12.19.23 to 12.13.24, and the operator stated that she would not be working until after the holidays. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include records of training required by the Department. Documentation showed late Fire System testing was conducted: 5.1.24 to 6.3.24, 7.2.24 to 8.2.24, 8.2.24 to 9.3.24, 11.1.24 to 12.2.24., which were beyond the 30-day testing requirement. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met.
