The chain link fence surrounding the facility's outdoor play area had several holes, and was in poor repair with sharp ends exposed presenting visible hazards. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Correction: The chain link fence was tied together with zip ties and tightly pulled together to remove all holes. Sharp ends were closed with pliers and wire. Inspection: Renewal.
