Observed a Bumbo in the infant room that has been recalled by the CPSC. The CPSC issued a repair (strap) for the Bumbo but the repair was not made. Requirement: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement. RECITED from 10/3/24: The loose fill under the embedded equipment on the playground does not meet the recommendations of the United States Consumer Product Safety Commission. The sand under the climber does not measure 9 inches consistently under the climber. It measures various depths. The sand that was placed at a few random areas to meet the 10/3/24 citation does not meet CPSP compliance guidelines. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: The climber in the playground will not be in use until enough sand is replaced to meet the required safety standards as per state regulations. DHS will be notified prior to the children using the equipment when the sand is poured, and a barrier will be in place to restrict the area until then. Inspection: Unannounced Monitoring.
