In the Middle Infants Classroom there was peeling paint behind the bookshelf next to the window. In the Middle Toddler Classroom there was peeling paint above the bookshelf beside the window. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: The fire detection system is monitored by our landlord and property manager. The file for Child #3, enrolled for more that 60 days at the facility, did not contain documentation of a 3rd dose of the DTAP and Pneumococcal vaccinations as required based on the child's age as per the ACIP recommended schedule. The file for Child #4, enrolled for more that 60 days at the facility, did not contain documentation of a 2nd dose of the DTAP, HIB, Pneumococcal, and Polio vaccinations as required based on the child's age as per the ACIP recommended schedule. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in childcare group settings). Child #3 and Child #4 must be dismissed from care by close of business 10/29/25 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. Correction: The updated form was submitted by the parent the next day. The file for Child #2 and Child #5 contained an emergency contact form that was not updated within 6 months of the previous update. Requirement: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: The child's emergency contact was in the file at the time of the inspection, although not returned within a 6 month period. In the Preschool Classroom there was a pillowcase that was ripped on three of the four corners and not in good repair. This pillowcase covered a vinyl pillow. Requirement: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement. At approximately 1:10 PM, the fire alarm in the facility was activated. During the evacuation it was observed that Staff #2 was supervising 8 children including 2 older toddlers and 6 young toddlers. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Correction: Teachers will not be permitted to leave the building without permission from the Director or Assistant Director to ensure that there are an adequate amount of staff on site at all times to ensure ratio requirements are met. Cert Rep observed 10 Preschool Aged children receiving care in the classroom, therefore leaving Child #9 not identified as being in an assigned primary group. Requirement: *Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Each teacher will be assigned a group of children along with a Transfer of Care sheet and identification tags. At approximately 11:00 AM, 9 children were observed receiving care in the Preschool Readiness 2 Classroom space that has a designated maximum capacity of 8. At approximately 1:10 PM, the fire alarm in the facility was activated. During the evacuation four staff who were on duty but were not providing childcare during naptime were observed reentering the facility premises after leaving the facility premise to purchase lunch. Due to the four staff leaving the premises, it was observed that Staff #2 was left out of ratio during the fire evacuation with 8 children including 2 older toddlers and 6 young toddlers. Requirement: Staff persons who are on duty but are not providing child care during naptime shall remain in the child care portion of the facility premises. Correction: Teachers will not be permitted to leave the building without permission from the Director or Assistant Director to ensure that there are an adequate amount of staff on site at all times to ensure ratio requirements are met.
