Observed foam blocks with bite marks. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Child #1 lacked child service reports completed at least every 6 months. Child #1 had child service reports completed on 9/1/24, 5/14/25, and 11/4/25. Requirement: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e). Observed a living room chair obstructing the archway between the front room and kitchen/eating area. Observed 2 pack 'n plays obstructing the archway between the front room and back room. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: PACKNPLAY WERE MOVED WHILE THE INSPECTOR WAS PRESENT, THE CHAIR WAS ALSO MOVED TO THE OTHER SIDE OF THE ROOM AND WILL REMAIN THERE GOING FORWARD Inspection: Renewal. Observed a soiled diaper being placed in a plastic bag instead of being discarded immediately by placing it into a plastic-lined, hands-free covered can. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: THE DIAPER WAS DOUBLE BAGGED IN AN EFFORT TO INCREASE THE SAFETY AND SANITIZATION AT THE FACILITY. REGULATIONS WILL BE OBSERVED IN THE FUTURE Inspection: Renewal.
