Disposable diapers (3270.135(a)(3))

In the Infant Room, the section of the diaper can which is designed to place the diapers, did not open without pressing on it, which then was not hands-free. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: Although we have a hands-free receptacle, it contains a flap to prevent odor from escaping. The infant diapers are not heavy enough to push the flap and fall into the receptacle on their own as they do in other classrooms. The flap was removed to allow diapers to be disposed of hands free. Child's hands washed (3270.134(a))

As confirmed by the staff in the Infant Room, infants' hands are not washed after being diapered. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: inspector did not watch staff change a diaper but asked to describe the procedure. She did not indicate that she washed the infant's hands. A diapering procedure poster was placed at the changing table and the proper diaper procedure reviewed with the staff member. Inspection: Renewal. Physician name, address, phone (3270.124(b)(2))

The Emergency Contact form, in the file for Child #3, did not contain the address or phone number of the child's physician or source of medical care. The Emergency Contact form, in the file for Child #4, did not contain the name or phone number of the child's physician or source of medical care. Requirement: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care. Correction: Our center has emergency contact information on both the emergency contact form and the agreement form. Although the information was provided and written on the agreement form, the parent did not write it on the emergency contact form. Inspection: Renewal. Admission date/Application, admission, withdrawal dates (3270.123(a)(6)/3270.182(2))

The Agreement in the file for Child #1, Child #2, Child #3, Child #4, and Child #5 did not specify the date of the child's admission. Requirement: An agreement shall specify the date of the child's admission. A child's record shall contain the dates of application, admission and withdrawal of the child. Correction: Our agreement form contains the wording "This agreement is effective (the start date of the child is entered) and both parent and director sign and date. We were advised that the wording needs to contain the words start date or enrollment date. The child start date is always this date and also in our software program and inside the child's file folder. A field on the bottom of the enrollment form labeled start date will now be used in the event the date the agreement is signed, and the start date are different. Inspection: Renewal. Staff assigned to specific children (3270.113(a)(1))

In Room 1A, the two (2) staff in the room were unable to accurately name the specific children in their supervision groups, who were an older toddler and preschool children, until the fourth attempt. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All child names are grouped by staff member both on a white board and the class schedule both posted in the classroom. The staff members were nervous and initially did not look at their lists when naming the children in the group. The staff members were advised that they were allowed to look at their lists and the proper names/ratios were then established. Inspection: Renewal. Refrigerator (3270.107)

In the unoccupied Preschool Transition Room, 3C, the plastic refrigerator container was not clean, and the operator confirmed that it was crayon markings. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The empty container (the containers was the same as used in the refrigerators) was accidentally placed in the refrigerator, and was removed. Inspection: Renewal. 2 feet apart (3270.106(f))

In Room 1B, the Young Toddler Room, none of the six (6) cots had 2 feet of space on three (3) sides of the occupied cots, during naptime. In the Infant Room, one (1) crib did not have two (2) feet of space on one (1) side, as it only measured 22". In the Infant Room, two (2) cribs did not have two (2) of space on two (2) sides, as one (1) side was up against a cabinet, and one (1) side was close to an unoccupied crib. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: The cot had 22" of space instead of the required 24", the cot was adjusted to meet the requirement of 24". Cribs were rearranged to allow 2 ft on 3 sides. Inspection: Renewal. Clean and good repair (3270.102(a))

In the Infant Room, a Becati, boppy pillow, contained torn Velcro and pulled stitching, which was not in good repair. In the Infant/Toddler Playground, a Lil Tykes playhouse was pulled apart between the tan and red panels, and was not in good repair. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Stairs, exits, etc. unobstructed (3270.91(a))

One (1) outdoor, toy storage cart, was in the path of egress from the exit located on Level E, from the staff room, and was obstructed. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: On this windy day, the wind blew an empty outdoor cart slightly in front of the exit door. The cart was replaced to its original spot and the wheel locks were applied to prevent it from being able to be moved in windy conditions which will prevent the door from being blocked. Protective Outlet Covers 5 yrs. or less (3270.65)

In Room 1B, the young toddler room, ten (10) protective outlet covers were missing on the cube, surge protector, which was located on the counter by the refrigerator, and were accessible to children five (5) years of age or younger. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: A new surge protector was recently purchased and on top of a counter. Not all plugs in the protector were filled, leaving open outlets. Space safe for large muscle activity (3270.62(c))

In the Preschool Playground, there were multiple dips in the surface, causing it to be uneven and creating a tripping/falling hazard. Requirement: Outdoor or indoor play space shall be safe for large muscle activity. Correction: Owner has contacted several turf companies to make necessary repairs, however, due to the glue products used and the process for repair, the air temperature must be 60 degrees or higher for 6 consecutive hours in order to make repairs. Health and Safety Training (3270.31(f))

The file for Staff #1 (see IS Code Sheet) contained documentation of Health & Safety training, Part 2, on 10.20.25, which exceeded 90 days of hire. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: Employee completed the wrong part two training (within the 90 days). This was not initially noticed, when it was, staff member immediately completed the correct training which was then after 90 days. Inspection: Renewal. General Health and Safety (3270.21)

In the Infant Room, a crib contained a loose, blue sheet, which was not fitted, posing a suffocation hazard. In the Infant Room, a child who had fallen asleep, was placed in the crib with a bib around his/her neck, posing a strangulation or suffocation hazard. In the Infant Room, a sleeping child was in the crib with a hat which had fallen off his/her head, posing a suffocation hazard. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: Parent provided a crib sheet that was too large. Crib sheet was removed and replaced with a clean crib sheet belonging to the center. Parent was advised of the proper size sheets and has now provided those. Bib and hat were removed. Transcript, diploma and letters (3270.192(2)(iv))

2nd Citation: Same citation previously cited on 12.31.24. The file for Staff #4 did not contain evaluated proof of education, and therefore, the educational equivalency was unable to be determined. Requirement: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. Correction: The staff's education documents were submitted to the evaluation company several weeks ago. We were given a 3-5 business day turn around time to receive the documents back. We have been contacted by the company multiple times for clarification, additional documents and addition al funds. We have provided the necessary info and payment and are still waiting for the return of the documents for the staff file. Our last follow up with the company was several days ago in which we were told an additional 3-5 days. We will continue to check in until documents are received or we will submit the request to another approved provider for the service. Inspection: Unannounced Monitoring. Age and Training/Exp, educ., training at facility (3270.31(e)/3270.192(2)(iii))

The file for Staff #1 contained documentation of 4.5 hours of child care training between 12.19.23 and 12.18.24, which did not meet the annual 12-hour requirement. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: Staff training was being tracked according to keystone stars. Staff had the proper amount of hours during the time frame of Sept 23-Sept 24. The correct time frame is 12/20/2023-12/19/2024, therefore not all of the trainings counted. The additional courses will be taken. The files for Staff #3 and Staff #4 did not contain evaluated proof of education, and therefore, the educational equivalency was unable to be determined. Requirement: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. Correction: Staff files contained translated copies of the foreign college degree, however, we were informed that the degree needs to be submitted to an agency to determine the US equivalency of that degree. The transcripts have been submitted to an approved agency to be evaluated for their equivalency. Once the agency completes its evaluation, the determination will be placed in the staff files. Now and in the future any foreign degree will be submitted to an approved agency to determine its equivalency and will be in the staff person's file. Inspection: Renewal. The file for Staff #1 contained documentation of Health & Safety - 1 Hour Update training on 9.18.23, which was due by 12.30.22, which exceeded the deadline. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: Self-corrected. Course taken. Fire safety - 1 yr. (3270.31(e)(4)(ii))

The file for Staff #1 did not contain any documentation of completed Fire Safety training in 2023 or 2024, at the time of the inspection, which is required at least every 12 months. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Training course was completed. Now and in the future, staff will complete all required trainings in the designated time frames. Inspection: Renewal. Protective Outlet Covers 5 yrs. or less (3270.65)

In Room 2D - SA/PK, a protective cover on the bottom electrical outlet was missing, which would be accessible to children 5 years of age or younger. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: The outlet cover was not replaced after a staff member used it for the vacuum cleaner. The safety cover was replaced over the outlet. The Health Assessment, in the file for Staff #2, did not contain a date, making it unknown as to when the Health Assessment was conducted, and is therefore, invalid. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: The staff member had the receipt and printout from the medical facility to prove the day she was there; however the actual form was not dated. The staff member took the form back to her provider to be properly dated and the form was placed back into the staff file.
