While testing the fire alarms during the inspection, the alarms were not interconnected (whereby when one goes off, they all go off) as required. Each of the 3 alarms had to be individually tested. There were 3 bottles that stay at the facility and are hand washed. They are not washed in a dishwasher, or sent home to be washed. Requirement: Disposable nursers shall be used for infants unless bottles are provided by the parent or unless a commercial dishwasher is used by the facility. Correction: Held a meeting with parents, parents agreed to provide me with 3 bottles a day and the bottles will be rinsed after use and sent home to be washed. Inspection: Renewal. A copy of the written 911 emergency transportation plan was not present in the vehicle used to transport children. Requirement: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children. The emergency contact form for Child #3 was not present in the vehicle used to transport school age children. Requirement: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion. Correction: Child #3 was no longer under my care following the inspection visit, therefore the emergency form for this child was filed away and not placed in the emergency bookbag that is taken along in the vehicle when transporting children. Child #1's Emergency Contact Form did not include the health insurance policy number. Requirement: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Seasonal coverings/Prohibit items in crib (3290.105(e)/3290.105(j))

There were no sheets provided for the 2 pack-n-plays or rest mats. Toys and a mobile were observed in a pack-n-play. Requirement: Seasonal, appropriate covering, such as sheets or blankets, shall be provided as agreed between the child's parent and the operator. Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib. Correction: Fitted sheets for rest mats and pack and plays were provided by provider, and toys were removed from inside the pack and play onto a safer area. Inspection: Renewal. Clean, age appropriate (3290.105(a))

The pack-n-plays and rest mats at the facility were not labeled for individual child use. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Peeling paint was observed on the baseboard heat cover and on the door frame between the 2 childcare rooms as well as under the gate. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. An open bag of Ice Melt (marked "Keep out of reach of children") was observed outside on the walkway used by children to enter/exit the facility, which was accessible to children. Requirement: Outside walkways shall be free from ice, snow, leaves, equipment and other hazards. Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Correction: I will remove the bag of ice melt from current location into a shed that is away and out of reach of children. Inspection: Renewal. Child #1's hands were not washed after the child's diaper was changed. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: I will observe staff and complete a handwashing checklist with staff and myself to ensure that children's hands are washed after diaper changes/toileting and before all meals and snacks.
