It was observed that there was a child's lunch box on table in the upstairs kitchen that contained yogurt and chicken that wasn't refrigerated. Requirement: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated. The two most recent health assessments on file for Staff Person #1 are dated 8/25/22 and 10/8/24, for Staff Person #2 are dated 8/16/23 and 10/24/25, for Staff Person #3 are dated 9/11/23 and 10/25/25, for Staff Person #6 are dated 2/23/23 and 9/19/25, and for Staff Person #7 are dated 2/20/23 and 9/22/25. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. It was observed that there was Infant Motrin and Tylenol in the medication box in the infant room that was lacking a medication log for the child that it was intended for. The emergency contact form for Child #1 was lacking the work addresses and telephone numbers of the enrolling parent. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: we had the parent of child #1 fill in the work address and telephone number on the emergency contact form. Inspection: Renewal. It was observed in the upstairs preschool room that there wasn't a handwashing sign by the diapering area. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. The first aid kit in the upstairs preschool room was lacking tape. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. The NSOR certificates on file for Staff #1 are dated 2/10/20 and 9/19/25, for Staff Person #2 and Staff Person #3 are dated 2/10/20 and 9/18/25, for Staff Person #4 are dated 2/10/20 and 9/12/25, and for Staff Person #5 are dated 8/10/20 and 9/12/25, which are all more than 60 months apart. The mandated reporter certificates on file for Staff Person #4 are dated 8/28/20 and 9/9/25 which are more than 60 months apart Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). The two most recent Pediatric First Aid/CPR training certificates on file for Staff Person #7 are dated 3/29/23 and 9/7/25, which are more than 24 months apart. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: In the future we will track all employee dates regarding their last First Aid/CPR training, thus enabling us to insure that all employee training does not exceed the 24 month window. The two most recent updates of the facility's emergency plan are dated 9/24/24 and 10/13/25, which are more than 12 months apart. Requirement: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility. Correction: Our revision date was 19 days past the annual renewal date of 09/24, 2024. I will update our cover sheet to align with 09/24/2024 on an annual basis. Inspection: Renewal. Stairs, exits, etc. unobstructed (3270.91(a))

It was observed that there were two strollers that were blocking the stairs to the egress from the lower level to outside. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: On the day of our inspection, 11/25/2024, the strollers were remove from the area bin front of the stair well, folded up and placed in our storage area. Inspection: Renewal. The file for Staff Person #2 contained Child Abuse Clearances dated 3/8/15 and 9/23/23 which are more than 60 months apart. The file for Staff Person #4 contained a State Police Clearance dated 10/22/24, which is after hire date (see LIS code sheet), and a Child Abuse Clearance which was obtained for volunteer purposes and is invalid for employment. The file for Staff Person #5 contained Child Abuse Clearances dated 5/14/18 and 10/11/23 and State Police Clearances dated 5/23/18 and 11/19/23 which are both more than 60 months apart. The file for Staff Person #6 contained Child Abuse Clearances dated 9/2/19 and 9/5/24 and State Police Clearances dated 9/6/19 and 10/3/24 which are both more than 60 months apart. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 4 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #4 may not work in a child care position at the facility. Correction: It was noted and recorded during our inspection on 11/25/2024 that a number of our staff ( #2, #4, #5, #6) had state police and child abuse clearances that reflected a period greater than 60 month between previous and current clearances and in the case of staff #4, contained a state police clearance dated after her hire date. Additionally staff person #4 had an abuse clearance for volunteer status. We submitted a request fro a new abuse clearance that will qualify # 4 for work/employment status at the daycare. Building Surfaces (3270.76)

It was observed in the upstairs school age room that there were wires that were exposed from a phone jack. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: During our inspection it was noted that there were exposed wires on one of our phone jacks. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
