Locked or inaccessible (3270.66(a))

The cupboard under the changing table in the infant room containing toxics labeled "Keep out of reach of children" was not latched, making the toxics accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The cupboard was immediately latched during the inspection. Staff will ensure that cabinets are locked at all times. Inspection: Renewal. Building Surfaces (3270.76)

There was a hole in the flooring in the preschool room, posing a potential tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. The agreement on file for Child #2 does not include the amount of the fee to be paid by the parent/guardian. Requirement: An agreement shall specify the amount of the fee to be charged per day or per week. Correction: The agreement was amended to include the fee amount to be paid by the parent. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). ·Child #1 must be dismissed from care by close of business on 10.9.25 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. There was teething gel in the Infant classroom which did not have written parental permission to administer. There was no lidded trashcan in the Preschool room where children were observed throwing away disposable cups after drinking water and where food remains were observed. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: We shall meet with all staff and review the garbage can regulation ensuring that lids of cans are always kept on. We will also inspect all garbage cans and replace any that might be loose or not secure enough to stay on. Inspection: Allocated Unannounced Monitoring. Handwashing signs (3270.82(h))

A staff person in the toddler area, where no source of running water is located for handwashing stated she uses hand sanitizer to clean the children's hands before eating and after diapering. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: We will review with staff that hand sanitizer is not an appropriate way to clean child/staff hands and the complete correct changing procedure and before eating. Name on bottle (3270.133(3))

There was a bottle of Tylenol in the young toddler area that was not labeled with the name of the child for whom the medication was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: The medication bottle was verified and labeled. Inspection: Allocated Unannounced Monitoring. While observing diaper changes in the Main childcare space, in the toddler diaper changing area, a staff person was observed opening the hands free can for diaper disposal with their hands. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: We reviewed proper Diaper Procedures with all staff, including the requirement to use the hands-free garbage can correctly. Inspection: Renewal. There were emergency contact forms in the Main childcare space for 3 children that could not be read, as the copies were too faint. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: We will recopy all Emergency Contact forms located in the main child care space and ensure they are legible. Inspection: Renewal. Refrigerator (3270.107)

There was no thermometer in the refrigerator in the infant room where infant bottles are stored. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: A thermometer was placed in the refrigerator in the infant room at the time of the inspection. Inspection: Renewal. Toilet and sinks proper height (3270.82(e))

The sink used for handwashing in the Main childcare space, in the young toddler area, did not include a stool for the children to utilize when washing their hands, but rather, a staff person was observed holding a young toddler over the sink to wash their hands. Requirement: Toilets and sinks shall be at proper heights for children using them or shall be easily approached by means of platforms or steps. Correction: Instead of storing a stool under the sink, we will move our larger stool from the Preschool room to the Toddler Room so it permanently stays at the sink and utilize a smaller stool in the Preschool room. We reviewed with all staff the proper diapering procedures that included having the children utilize the stool and not holding them over the sink for hand washing. Inspection: Renewal. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: We will ensure to call and check the status of every applicants NSOR application 14 days after their application was submitted to ensure it is processed and we receive it on time. In addition, if we do not receive a NSOR certificate for a staff member within the 45 day probation period, that staff person will be removed from caring for children until it is received.
