SECOND CITATION; PREVIOUSLY CITED ON 4/1/26: During rest time, the following pieces of rest equipment were observed in use with less than 2 feet of space on three sides: In the Toddler room, there was a mat in use placed against the wall and against a piece of furniture with less than 2 feet of space on a third side between the mat and another piece of furniture. In the Pre-K room, there was a mat in use in between two tables, only allowing for 2 feet of space on two sides. In the Preschool 2 room, there was a mat in use with less than 2 feet of space between the mat and the wall on one side, and shelves on two other sides, only allowing for 2 feet of space on one side. Requirement: 1. At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. THIS PORTION OF THE PLAN SHALL HAVE AN IMMEDIATE CORRECTION DATE. 2. The director(s) shall work with classroom staff to develop a pictorial layout of each classroom (i.e. a "map") for naptime, to include positioning of stationary staff, furniture placement, and spacing of rest equipment. The layout must allow for the maximum number of children present in each room at naptime and provide for two feet of space on three sides of all pieces of rest equipment while in use, unless placed in a corner of two walls where 2 feet of space would be required on the remaining two sides. These pictorial layouts shall be submitted to the certification representative and posted in each classroom. THE OPERATOR SHALL PROVIDE A DATE FOR WHICH THIS PORTION OF THE PLAN WILL BE COMPLETED. Correction: 1. 5/4/2026- The positioning of all sleep mats in the school were adjusted to ensure that there was at least 2 feet of space that is required on three sides of the mat. 2. 5/12/2026- The management team went into each classroom to create a diagram to show how furniture and sleep mats should be arranged to ensure that there is at least 2 feet of space on three sides of the mat. These diagrams were then distributed in the classrooms and placed in the classrooms to allow for future reference if needed. In addition, the health and safety coordinator on site will help the management team conduct routine checks to ensure proper mat spacing. Inspection: Unannounced Monitoring. Correction: Spacing of both the cribs and sleep mats were reevaluated and new room arrangement for naps were established to maintain at least 2 feet of space on three sides of the mats and cribs. There was an area of damaged plaster on the wall in the the Preschool 2 room behind a trash can to the right of the door leading to the Pre-K Room. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. In the Preschool 1 room, there was an unlocked cabinet with bleach water and cleaning supplies on a low shelf in reach of children. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. There were no emergency contact forms in the infant room for the children receiving care in that space. According to staff statements, the set of emergency contact forms in the classroom was the one that was taken outside when a staff person took some of the children outside. There were no emergency contact forms remaining in the room while a group of staff and children remained in the classroom. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: Additional copies of emergency contact forms were placed in the outside backpacks. Teachers were then informed that if there group were to split up, one teacher must take the binder that has emergency contact forms for the children and the other teacher must have the outside backpack. The current firesafety trainings completed and documented on file for Facility Persons #2 and #3 were each completed more than 12 months after the pervious firesafety training for the respective staff person. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Teacher fire safety trainings for Faculty Persons #2 and #3 were completed at a date past their previous 12 months after the previous fire safety training. Inspection: Renewal. According to staff statements, the first aid kit from the classroom was the one that was taken outside when a staff person took some of the children outside. Correction: Additional first aid kits were placed in every classroom. This allows for first aid kits to be available to all children in all learning environments (inside and out). There was rust on a pole in the corner of the preschool outdoor playspace. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Pole was sanded where rust was evident. The pole was then painted with a protective coat. Black tape was also added to the pole to reinforce the barrier. Inspection: Renewal. There was a cot in use placed in front of a door leading from the Toddler to the Toddler Transition room, obstructing the exit. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: Room arrangement was reevaluated in the toddler classroom. To make sure the classroom entry door was clear, additional furniture was moved to create able sleeping space for the children while still not blocking the toddler room entry. Inspection: Renewal.
