There was a blister pack of a child's Zyrtec tablets , not in the original container, in the Schoolage room. Requirement: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. Correction: The zyrtec tablets were removed from the the medicine box in the school age room. The parents were notified that the medicine will be sent home and when they bring new medicine in, it will need to be in the original container. Inspection: Renewal. Immunization record/Dismissal policy (3270.131(d)(5)/3270.131(e)(3))

The immunization record on file for child #1 indicated the child did not have sufficient doses, as per the ACIP recommended immunization schedule for a child of their age, of the following immunizations: hepatitis B, DTAP, Hib, pneumococcal and polio. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Child #1 must be dismissed from care by close of business on 6/10/26 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter Correction: We contacted the parents of child #1. We obtained a letter from the parents that they are choosing to exempt their child from immunizations due to personal beliefs. They also obtained a doctor note explaining the delay of doses for medical reasons. The notes were put in child #1's file prior to her returning for care. Inspection: Renewal. The NSOR clearance on file for Facility Person #1 was completed after her start date in a child care position. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. There was a child's medication in the Preschool 1 room not labeled with the child's name. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: We wrote the child's name on their medication. Inspection: Allocated Unannounced Monitoring. There were 2 rest mats in use in the Toddler 2 room with less than 2 feet of space on 3 sides. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: We shifted furniture around to ensure that there was 2 feet of space on 3 sides of the naps. Inspection: Allocated Unannounced Monitoring. There was peeling paint/damaged plaster on the wall behind a trash can in the Infant room. There was an uncovered outlet on the wall in the hallway, accessible to children. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: The outlet was covered immediately when discovered. The agreement on file for child #1 was the original. Requirement: A parent shall receive the original agreement. The facility shall retain a copy of the agreement. There were two packnplays in the infant room with loose sheets. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.
