Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1- may not work in a child care position at the facility. Correction: Staff #1 was unable to come to work until the FBI clearance was updated. Staff got finger prints take in June 2025 but never received her clearance. Staff member was notified that it was missing and then went to the fingerprint place to have it done again. Building Surfaces (3270.76)

In the indoor toddler gross motor space, it was observed that there were missing ceiling tiles from the ceiling. In the gym it was observed that the protective mats around the stage were peeling. In the outdoor preschool and toddler play spaces it was observed that the poured surface is deteriorating and had multiple holes and open gaps leaving it in not good repair. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Toddler gross motor space ceiling tiles were replaced immediately 10/21/25. Gym mat was taped 10/21/2025. Playground was closed and will be taken off our license until further notice. 10/21/2025 Spring 2026 will most likely be the opening for the playground. Inspection: Renewal. In the boy's bathroom in the infant/toddler hallway it was observed that the garbage can was lacking a lid. Requirement: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle. Loose mulch, less than one inch in diameter was observed accessible to toddlers in the Outdoor Play Area. In the infant outdoor play area, there is a tarp under a plastic cube climbing unit that is beginning to shred into tiny plastic strips. Requirement: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care. Correction: BCPCC plans to correct this violation by having our outdoor playground contact come and reseal the mulch to ensure that it is not loose. Inspection: Renewal. Clean and good repair (3270.102(a))

There is a wooden frame hanging on the fence of the outdoor play area that contains a sharp piece of tin and the frame is splintering. In Room 202, the board books are ripped on the corners. In Room 234, the plastic measuring basket for the scale is broken/cracked. In Room 203, there is a hole in the tall cabinet on the wall with the door which presents a pinching hazard. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: BCPCC plans to correct this violation by removing the wooden frame hanging on the fence in the preschool outdoor play area. The board books with ripped corners in room 202 have been removed. The plastic measuring basket for the scale has been removed from room Room 234. No peeling paint or plaster (3270.77(a))

In Room 204 there is peeling paint on the tall cabinet. In the outdoor play space, there is peeling paint on the metal car frame and the metal tunnel. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Tape was used to cover the damaged surface area on the tall cabinet in Room 204 to protect against peeling paint. Building Surfaces (3270.76)

In the outdoor preschool and toddler play areas, the playground unitary protective surface had large cracks and holes in it creating a tripping hazard. In the outdoor infant play area, there is a sharp rock near the basketball hoop. In Rooms 232 and 234 the wall covering under the soap dispensers is damaged and the corners of the drawers/cabinets along the sink counter have rough, sharp edges. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: BCPCC plans to correct this violation by filling the holes in the outdoor play surfaces. BCPCC contacted a company to fill the holes and they came out on 10/18 to repair tripping hazards. BCPCC contacted buildings and grounds to remove or cover the sharp rock in the infant outdoor play area. In Rooms 232 and 234 the wall coverings under the soap dispensers were covered with contact paper - this citation was corrected day of. Inspection: Renewal. There were 6 windows in the Indoor Play Area above the ground floor that lacked screens and were not limited to opening 6 or fewer inches. There were 2 windows in Room 232 above the ground floor that were lacking screens and were not limited to opening 6 or fewer inches. Requirement: Windows or doors used for ventilation shall be screened when open. Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: BCPCC plans to create a sign next to each window that will state "Do not open per DHS regulation § 3270.72 windows above the ground floor shall have screens in good repair and be limited to opening no more than 6 inches. Staff Person #4 may not work in a child care position at the facility. Correction: BCPCC plans to correct this violation by ensuring all staff members complete the NSOR and proper fingerprinting clearance before the date of hire. Staff Member #1 and Staff Member #2 have already corrected this violation and now have current and accurate NSOR clearances and fingerprinting clearances. Small Toys and Objects (3270.103)

In Rooms 215 and 213, there are foam blocks with teeth marks in them. In Room 207, the vinyl is peeling on the foam climber. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: BCPCC plans to correct this violation by removing the foam blocks with teeth marks from room 215 and room 213. Play with foam blocks will be closely monitored to ensure children are engaging appropriately with the materials. In room 207, the peeling vinyl on the foam climber was rectified using duct tape.
