The emergency contact information for child #1 did not include parental consent for emergency medical care. During the renewal inspection on 9/24/25, the updated letter to parents regarding the emergency plan could not be located. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures described in subsection (a). The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: The letter to parents about the emergency plan will be updated to include accommodations for infants and toddlers, children with special needs, and children with chronic medical conditions. Every 60 days (3290.94(a)(1))

During an unannounced inspection on 5/20/25, the most recent fire drill was dated 12/20/24. Requirement: The operator or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days. Refrigerator (3290.106)

The thermometer in the refrigerator was an oven thermometer, therefore, the temperature of the refrigerator could not be determined. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. Outdoor equip.- protective surfacing (3290.102(c))

There was insufficient mulch to cover the fall zones around the entire embedded play structure. Requirement: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Correction: We will not use the playground until the mulch can be installed at correct depths. Requirements (3290.74(a))

There was deck board that was loose and sticking up. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. There was no state police clearance, child abuse clearance, NSOR or signed disclosure statement on file for household member #1 who is over the age of 18. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: All clearance info was sent out and forms completed.
