A large area of peeling paint was observed on the wall above the baseboard in the hallway located between the Infant 1 and Infant 2 rooms. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. In the Preschool/SA Play Space, a children's play slide was observed to be cracked and not in good repair. In the Preschool/SA Play Space II, three children's playground structures were observed to be cracked in multiple areas. These conditions expose the children in care to rough edges, sharp corners and pinch points. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The salvageable playground equipment was repaired and free from rough edges. The pieces that could not be mended were removed from the area. Inspection: Renewal. In the Main room and the Older Preschool-2 room, soiled tissues were observed contained in a plastic-lined receptacle but did not have a closed lid. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: The receptacle in the Main room is now labeled for 'Recycling Only.' A new hands-free lidded bin has been placed in the Older Preschool 2 room. Inspection: Renewal. The file for Staff Person #1, who has lived in the state of South Carolina within 5 years of their date of hire (See IS Code Sheet), did not contain documentation of the state's Child Abuse, Sex Offender, and Criminal History clearances as required under CPSL to work in a child care position. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person #1 may not work in a child care position at the facility. Correction: The staff person was removed from a childcare position and re-applied for the necessary out-of-state clearances. Inspection: Renewal. A bottle of Equate Pain Reliever, labeled "Keep out of reach of children," was observed in an unlocked drawer of a staff person's desk accessible to children in care in the Older Preschool-2 room. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The bottle was immediately removed from the desk in the classroom. The two most recent fire safety trainings in the file for staff person #1 are dated 6/8/23 and 9/3/24, which are more than one year apart. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Staff person was on leave when the yearly fire safety was due. Upon return to employment at our center, the training was completed. Inspection: Renewal.
