Training regarding plan (3270.27(c))

The file for Staff #2 contained documentation of Emergency Plan Training on 8.26.24 and 9.2.25, which exceeded the annual requirement. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: The staff completed the Emergency Plan training when she returned from a vacation. In the future, the staff member will complete the training prior to the expiration date, even with vacation or time off planned. Inspection: Renewal. Comply with CPSL (3270.32(a))

The file for Staff #1 did not contain documentation of required out-of-state clearances, since the staff member did not know if the out-of-state residency was within five (5) years. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Staff #1 may not work in a child care position at the facility. Correction: Staff #1 is not working at the facility until the out-of-clearances are received. The staff member is unsure of the exact date which she moved into the state and cannot confirm if it has been over the 5-year window. The staff member has applied for and is waiting for the return of the out of state clearances for North Carolina required by the Department of Human Services in Pennsylvania. After multiple attempts by the provider, Staff #1 has not contacted the provider, and is therefore no longer employed at the facility. Inspection: Renewal. Protective Outlet Covers 5 yrs. or less (3270.65)

In the Main Room, a protective receptacle cover was missing from the top outlet on the back divider wall, which was accessible to children 5 years of age or younger. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: The electrical outlet missing the receptacle cover is 3.5 feet on the divider wall by infant area. The receptacle cover was placed on the outlet and provider will ensure that the outlets in all areas of the facility accessible to children 5 years or younger, will maintain a receptacle cover. Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports (3270.131(b)(2)/3270.182(1))

The file for Child #1, an older toddler, contained a Health Assessment which was dated more than 12 months ago. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: The provider requested and obtained a copy of a health assessment for child #1 which is within the 12-month period which the most recent health assessment was due. Inspection: Renewal. Arrival/departure times (3270.123(a)(4))

The Agreement, in the file for Child #1, did not specify the child's departure time. Requirement: An agreement shall specify the child's arrival and departure times. Correction: The provider completed the departure time on the child's file to state "varies", this child is enrolled in ELRC subsidy program and is approved for part-time and full-time days making her departure time different based on the day.
