The parent letter explaining the emergency procedures did not include information about lockdown or accommodations for infants/toddlers. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures described in subsection (a). The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: The information on lockdown and accommodations for infants/toddlers will be added to the the letter. The letter will continue to be handed out at enrollment. Inspection: Renewal. The work phone number for child #2, #3, and #4 was missing from the emergency contact information. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Disposable cups were being used and not discarded after each use. Requirement: Disposable cups, plates and eating utensils may be used if discarded after each use. Child #1 was placed to sleep in an infant bouncy seat placed inside a pack and play. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Correction: Child will not be placed in bouncy seat to sleep. The most recent child abuse clearance for staff/household member #1 is dated 7/6/20. The most recent child abuse clearance for staff/household member #2 is dated 7/21/20. The most recent child abuse clearance for staff/household member #3 is dated 7/21/20. There was no state police clearance on file for staff/household member #4. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Written reports of accidents, injuries and illnesses at the facility were not being provided to parents, retained in the child's file or retained in an accident file. Requirement: A child's record must contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. A written plan which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency was not posted at the facility. Requirement: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children. Correction: I will write up a plan stating what will happen in the instance that a child needs to be transported for emergency care and post it on the Bulletin board. Inspection: Renewal. The emergency contact information for child #1 did not include the health insurance policy number. Requirement: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.
