    During the investigation, it was verified through staff interviews and record review, that Staff #1 fell asleep in the classroom for approximately five minutes, while they and another staff person were responsible for helping the children prepare for naptime. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Correction: The staff member involved was counselled and received a written reprimand that was signed and filed 10/24/25. Inspection: Complaints- Legal Location.     Child #1 was left in room during a fire drill on November 21st, 2025, for approximately 47 secs from the reviewed video. Child was noticed by staff from the next room to the Young Toddler Room was exiting, as protocol calls for the adjacent room staff look through window to verify no one is left behind and administration does a final sweep. Staff from the child's room were enroute back as they noticed their count was off before exiting the building. Child #1 was in the Primary Care group of Staff #1. Staff #2 and #3 were also in the room. Requirement: TIERED IS: 1.Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. 2. Administrative staff will conduct a staff meeting and review the fire drill procedures for all staff and provide the staff meeting minutes and sign in sheet to the certification representative upon completion of the meeting. Correction: Review of fire drill procedures is conducted with new hires within 2 days of employment as well as annually with all staff, typically in September at an all-staff meeting. Supervision policy was reviewed with staff #1, #2, and #3 that were directly involved as well as a review of fire drill procedures. Inspection: Complaints- Legal Location.     On 9-15-25 Staff #1 admitted to using a Vape Pen inside and outside the facility and in the presence of children. Requirement: Cigarettes, pipes or cigars may not be smoked in a child care space, a play space or a food preparation area when children are in care or when food is being prepared. Correction: An incident of an employee vaping in the classroom was reported to administration on 9/15. The employee was met with on 9/16/2025 and they were terminated from the program immediately due to their actions. Inspection: Complaints- Legal Location.
