In the Toddler One's and Two's Room, one ceiling tile near the doorway was observed to not be kept clean as evidence by having water damage spots from an unknown water source. In the Movement Matters Zone, three ceiling tiles were observed to not be kept clean as evidence by having water damage spots due to a leak from the roof. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Moving forward the Center Director will make sure floors, walls, ceiling tiles and other surfaces, including the facilities outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Protective Outlet Covers 5 yrs. or less (3270.65)

In the Kindergarten Prep Room, an electrical outlet accessible to children 5 years of age or younger that was located near the thinking and learning area was observed to be missing one protective cover. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Moving forward the Center Director will walk through the center to make sure that all electrical outlets have a protective receptacle covering on them in each classroom and hallway of the building where children ages 5 years of age or younger have access to. The staff file for Staff person #1 was observed to contain a current State Police clearance dated 1/22/26 and there was no proof of having applied for the clearance prior to first day working with children (See IS CODE SHEET) as required under the Child Protective Service Law (CPSL.) A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. The staff file for Staff person #1 contained a current health assessment that lacked the physician's or CRNP's assessment of the person's suitability to provide childcare. Correction: Staff person # 1 provided an updated health assessment with a physician's (or CRNP's) assessment that staff person was suitable to provide child care. Correction: The Center Director contacted the facilities department and placed a work order immediately to replace the ceiling tiles that were not in good condition. Inspection: Renewal. Correction: The Center Director immediately got covers for the electrical outlets and they were placed immediately in the hallway outlet across from the entrance to the Toddler 2's room and the Preschool room's thinking and learning area. Staff person #2 was also observed to have a West Virginia Sex Offender check in the file however it was not dated therefore it cannot be determined if the check was done prior to working with children. The file for Staff person #5 (See IS Code Sheet), who has resided out of state within the previous five years, was observed to lack a Georgia Sex Offender check. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Moving forward the Center Director will make sure that all staff and volunteers participate at least annually, in fire safety training conducted by a fire protection professional before the training expires. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of the facility persons during a fire drill, and during a fire, and the use of the facilities fire extinguishers, not including discharge of the fire suppressant agent.
