Until such time as the required training has been completed, staff person #3 must be supervised, when interacting with children at a minimum by, an AGS or higher who has completed all preservice trainings and has all qualifications to care for children unsupervised. Medication shall be administered to only the child whose name appears on the container. Correction: The antibiotic cream and alcohol wipe items were residuals of the new supplies purchased to replenish the Emergency Bag. The items were removed immediately from the Emergency Bag. Correction: Contacted the building owners about this damage that took place with their remodeling of the 2nd floor above the center. The hole was repaired immediately. This presents a potential choking hazard for children who may still be placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. On 4/16/23 at 10:30 AM, Staff Person #2 was supervising 8 Preschool and 2 YSA children. Staff Person #2 identified the 8 Preschool children. Staff Person #2 was unable to name the two YSA children. At 1:30 PM, Staff Person #2 was supervising 8 Older Toddlers who were napping. Staff Person #2 identified 7 of the Older Toddlers. Staff Person #2 was unable to name one of the Older Toddlers. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
