The file for child # 1 a preschooler contained a current health report dated 3/5/25. The report was obtained after 12 months the prior report was dated 2/27/24. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Clean and good repair (3270.102(a))

There were 2 cracked clear bins in the preschool room. There were 2 cracked blue bins in the cubby's upstairs in the hallway. There was a broken wihite hamper basket in the the hallway. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. The sealant silicone around the pre-k room bathroom sink was damaged making the sink feels lose. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. There was 1 uncovered outlet accessible to children 5 years of age or younger in the preschool room. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Clean and good repair (3270.102(a))

There were 2 cracked bins in the Pre-K/SA room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The director removed one bin and threw it away. It will no longer be in the facility/accessible to children. The second bin was able to be repaired using tape. The entire crack is covered with tape, and poses no threat to the health and safety of the children. Inspection: Allocated Unannounced Monitoring. The laundry room door was malfunctioning, the door does not stay closed making the laundry room accessible to the children. The entry hallway closet door hook latch was broken. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: A latch was installed on the laundry room door to make sure it remains closed and inaccessible to children at all times. There was an unlocked closet door that contains different cleaning materials like, Clorox, disinfecting wipes, Paint cans and cleaning sprays, labeled " Keep out of reach of children" on the hallway across from the infant room accessible to children. There was an All free and clear mighty pacs can labeled " Keep out of reach of children" in the 1st bathroom to the left across from the OT room accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The door to this closet will remain latched at all times. All cleaning/toxic materials will be kept in these locked closets. General Health and Safety (3270.21)

There was an exposed screw sticking out in the Pre-k/SA room cubbies. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The screw was removed from the cubbies, and tape was placed over the hole. Any splintering wood from the screw has been covered. Inspection: Allocated Unannounced Monitoring. According to the Fire Detection System Test Log, the tests were greater than 30 days apart from3/15/24 to 4/15/24 and 5/14/24 to 6/14/24. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.
